Özet

AMAÇ; Behçet olgularında, hastalık aktivitesi ve trombotik komplikasyonlar için risk faktörü olarak lipoprotein(a) [Lp(a)] plazma konsantrasyonlarının değerlendirilmesi METOD: Çalışma grubu 30 Behçet Hastasından (8 aktif, 22 inaktif), kontrol grubu 30 sağlıklı kişiden oluşturuldu. İnaktif Behçet olgularının 7'sinde geçirilmiş trombotik komplikasyon mevcuttu. Hastalık aktivitesi oral aft, genital ülserasyon, üveit ve vaskülit gibi klinik bulguların yanı sıra Lp (a), lökosit, C Reaktif Protein, kompleman C3, kompleman C4 gibi akut faz reaktanları da değerlendirilerek belirlendi. BULGULAR: Plazma Lp(a) ve diğer akut faz reaklanlarının konsantrasyonlarında, kontrol grubuna göre çalışma grubundaki yükseklik istatistiksel olarak da anlamlı bulunmuştur (p 0.05). SONUÇ: Lp (a) düzeylerindeki artışın diğer aktif faz reaktanları ile korele olması, Lp (a)'nın yüksek plazma konsantrasyonlarının hastalık aktivitesi için bir risk faktörü olabileceğini düşündürmektedir. Yüksek Lp (a) düzeyinin, trombotik komplikasyonların gelişiminde bir risk faktörü olduğunu söyleyebilmek için daha geniş hasta gruplarında, trombotik komplikasyonlar aktif ve inaktif iken yapılan ölçümlerin karşılaştırılması gerektiği kanısındayız.

Abstract

PURPOSE: To evaluate plasma concentrations of lipoprotein (a) [Lp (a)] as a risk factor for disease activity and thrombotic complîcations <rt Behcet's disease, METHODS: Thirty patients with Behçet's disease (8 active, 22 inactive) as study group and 30 normal subjects as control group were included in this study. Seven patients of inactive Behçet's disease group had the medical history of thrombotic complications. The disease activity was evaluated by clinical manifestations such as oral aphthous lesions, genital ulcerations, uveitis and vasculitis, and laboratory investigations such as leucocyte count, Lp (a), C Reactive Protein, compleman C3 and compleman C4 concentrations. RESULTS: Plasma Lp (a) and other acute phase reactant concentrations were significantly higher in the study group than in the control group (p<0.01). These concentrations were also higher during the active period of the disease than during the inactive period of the disease (p<0.01). Lp (a) concentrations showed significant correlation with concentrations of other acute phase reactans. There was no significant statistical difference between the patients with and without thrombotic complications for any of these measurements (p>0.05). CONCLUSION: The correlation between the Lp (a) and other acute phase reactan concentrations leads us to suggest that high plasma levels of Lp (a) might be a risk factor for the disease activity of Behçet's disease. To be able to say that high plasma Lp(a) concentrations might be a risk factor for the thrombotic complications in patients with Behçet's Disease, the same measurements should be done in large series during the thrombotic complications are in active and inactive periods and then compared.
