Şaşılıkta reoperasyon gerektiren olguların sonraki ameliyat başarılarının incelenmesi

Özet
AMAÇ: Ezotropya (ET) ve ekzotropya (XT) nedeniyle ameliyat edilip reoperasyon gerektiren olguların 2. ve 3. ameliyat başarılarının incelenmesi YÖNTEM: Otuz yedi ET ve 9 XT olgusu çalışma kapsamına alındı. İlk tanısı ET olan 37 olgunun 33'ü (%89.1) rezidüel ET, 4'ü (%10.B) ardıl XT nedeniyle tekrar ameliyat edilirken; ilk tanısı XT olan 9 olgunun 8'i rezidüel XT nedeniyle tekrar ameliyat edildi. Birinci ameliyatta ET'li olguların 4'üne tek horizontal kas, 32'sine iki kasa, 1 olguya da üç kasa müdahale uygulandı. İkinci ameliyatta 21 olguda tek kasa müdahale edilirken, 16 olguda iki kasa müdahale uygulandı. Birinci ameliyatta XT'li olguların 9'una iki kas cerrahisi, ikinci ameliyatta iki olguya tek kas, 7 olguya iki kas cerrahisi uygulandı. BULGULAR: İlk ameliyat öncesi ET'li grupta ortalama deviasyon 42.0 pirizm dioptri (PD) iken XT'li grupta ortalama 38.75 PD idi. ET'li olguların 30'unda (%81.8) ikinci ameliyat sonunda başarı sağlanırken XT'li olguların 8'İnde (%88) 2. ameliyat sonunda başarı sağlandı. Ameliyata bağlı düzelme esas alınarak yapılan değerlendirmede ikinci ameliyatın ET'li ve XT'Ii gruplardaki başarısı arasında anlamlı bir fark yoktu (p=0.707). ET'li grupta birinci ve ikinci ameliyatların başarıları arasında anlamlı bir fark bulunmazken, XT'li grupta birinci ameliyat sonundaki başarı ikinci ameliyata göre anlamlı ölçüde yüksekti (p=0.0382). İlk ameliyat öncesi görme seviyesi ölçülen 24 olgunun 13'ünde ambliyopi mevcuttu. İkinci ameliyat sonrası başarı elde edilemeyen olguların tamamı ambliyoptu. SONUÇ: Klasik miktarlarda uygulanan şaşılık ameliyatlarında ET ve XT olguları arasında cerrahi başarı yönünden fark olmadığı, cerrahi başarısızlık gösteren olgularda ambliyopi insidansının yüksek olduğu gözlendi.
Abstract
PURPUSE: To observe the success rate of reoperations on eyes which were previously operated for esotropia (ET) and exotropia <XT). METHOD: Thirty-seven ET and 9 XT were included in our study. Among 37 eyes which were previously diagnosed as ET, 33 (%89.1) were operated for residual ET, 4 (%10.8) were operated for consecutive XT. Eight of the 9 eyes which were operated for residual XT were previously diagnosed as XT. During the first operations of the ET cases 4 eyes were operated on 1 horizontal muscle, 32 eyes were operated on 2 muscles and 1 patient operated on 3 muscle. At the second operations, 21 eyes were operated on 1 horizontal muscle and 16 eyes were operated on 2 muscles. All of the XT cases had undergone surgery of 2 muscles in the first operations and 2 eyes had undergone surgery of 1 muscle and 7 eyes had undergone surgery of 2 muscles at the second operations. RESULTS: The mean preoperative deviation was 42.0 prism diopters (PD) in the ET group and 38.75 PD in the XT group. The postoperative success was achieved in 30 of ET (81.80%) and 8 of XT (88.00%) cases. According to the deviation corrected by operation the success rate of the second operations was not significantly different in eyes with ET and XT (p=0.707). There was no significant difference between the success rates of the first and second operations in eyes with ET. The first operations were significantly more successful than the second operations in eyes with XT (p=0.0382). Thirteen of the 24 whose preoperative visual acuity could be determined were amblyopic. The unsuccesful cases after the second operation were all amblyopic. CONCLUSION: There was no significant difference between the success rates of classical type and amount of surgery in ET and XT cases. The incidence of ambliyopia seem, to be higher in cases who had unsuccessful surgical results.
