Yüksek miyopik olgularda bikonkav iris-claw lens implantasyonu beraberinde fotorefraktif keratektomi

Özet
AMAÇ: Bikonkav iris-claw göz içi merceği uygulamasını takiben, kalan kırma kusurlarını düzeltmek için fotorefraktif keratektomi (PRK) yapılan yüksek miyop hastalarda kombine cerrahinin sonuçlarını değerlendirmek GEREÇ VE YÖNTEM: On yedi diyoptri (D) ve üstü miyopisi olan beş hastanın 10 gözünü içeren çalışmada, fakik intraoküler iris-claw lens implantasyonu sonrası, tüm gözlere kalan rezidüel miyopi ve/veya miyopik astigmatizma için, ortalama postoperatif 6. ayda PRK uygulandı. Hastalar ortalama 24.7± 8.68 ay süre ile izlendi. Her iki girişim öncesi ve sonrası sferik ekivalan refraksiyon ve görme keskinliği değerleri arasındaki değişim ANOVA testi kullanılarak karşılaştırıldı. Gruplar arası karşılaştırmalar için ise Tukey çoklu karşılaştırmalar testi kullanıldı. BULGULAR: Fakik intra oküler lens implantasyonu öncesi, en iyi düzeltilmiş görme keskinliği (BCVA) ortalama 0.3± 0.15 iken, operasyon sonrasında BCVA ortalama 0.35 ± 0.16 'e ulaştı. Düzeltilmemiş görme keskinliği (UCVA) PRK sonrası altıncı ayda ortalama 0.41± 0.15, bir yıl sonunda ise ortalama 0.40 ± 0.17 'e yükselerek anlamlı artış gösterdi. Takip süresi sonunda hastaların % 60'nda görme keskinliğinde artış oldu. Birinci operasyon öncesi ortalama sferik ekivalan refraksiyon -21.28 ± 2.12 D, iken operasyon sonrası ortalama -3.34 ± 1.46 D, PRK sonrası altıncı ayda ortalama -0.64 ± 0.49 D, birinci yıl sonunda ortalama -0.80 ± 0.47 D olarak belirlendi. Gözlerin % 80'i takip süresi sonunda, ± 1 D sınırındaydı. SONUÇ: Fakik iris-claw lens beraberinde PRK uygulaması, yüksek miyop olguların tedavisinde etkin bir seçenektir.
Abstract
PURPOSE: This study was designed to evaluate the results of combined surgery in high myopic cases, that photorefractive keratectomy (PRK) was performed to correct the residual refractive error after the implantation of phakic iris-claw lens. MATERIALS AND METHODS: Ten eyes of five patients with high myopia (-17,00 D or more) were included in this study. PRK was performed aproximately 6 months after the phakic intraocular lens implantation, for the correction of residual myopia or myopic astigmatism. Mean follow-up time was 24.7± 8.68 months. Repeated measures of ANOVA test was used to analyze the differences of the spherical equivalent refraction and the visual acuity before and after the both procedures. Tukey's multiple comparisons test was used to compare the results between the groups. RESULTS: Best-corrected visual acuity (BCVA) was 0.3± 0.15 before the PIOL implantation and became to 0.35± 0.16 after the operation. Uncorrected visual acuity (UCVA) significantly increased to 0.41 ± 0.15 after six months and to 0.40 ± 0.17 after a year following the PRK. Visual acuity improved in 60% of all patients at the end of the follow-up time. Mean spherical equivalent refraction was -21.28 ± 2.12 diopter (D) before the first operation and it became -3.34 ± 1.46 D after the operation. Six months and one year after PRK, spherical equivalent refraction was determined as -0.64 ± 0.49 D and -0.80 ± 0.47 D respectively. Finally, 80% of the patients were in ±1D of emmetropia. CONCLUSION: Combined surgery of biconcave Iris-claw lens implantation and photorefractive keratectomy is an effective alternative for the treatment of high myopia.
