Mini-nük tekniği ile ameliyat edilen 84 olguya ait sonuçlar

Özet
AMAÇ: Manisa SSK Hastanesinde Ocak 1999- Mart 2000 tarihleri arasında mini-nük tekniği ile katarakt ameliyatı yapılan 84 gözde, intraoperatif ve postoperatif karşılaşılan komplikasyonlar ve görsel sonuçlar değerlendirildi. GEREÇ VE YÖNTEM: Subtenon anestezi altında Blumenthal'in mini-nük tekniği uygulandı. Vakalarda skleral tünele sütür konulmadı. Ameliyat sonuçları görme değerleri, komplikasyonlar açısından incelendi. BULGULAR: Gözler ortalama 5.5 ay (1-12 ay) süre ile takip edildi. İntraoperatif 4 gözde (%4.8) arka kapsül rüptürü, 2 gözde (%2.4) vitre kaybı, 2 gözde (%2.4) iris prolapsusu, 3 gözde (%3.6) intraoküler lens yerleştirilirken hemoraji, 2 gözde (%2.4) iridodiyaliz ve 1 gözde (%1.2) skleral tünelin üsten perfore olması meydana geldi. Postoperatif 15 gözde (%17.8) geçici kornea ödemi, 1 (%1.2) gözde büllöz keratopati, 8 gözde (%9.5) arka kapsül kesafeti, 1 gözde (%1.2) glokom, 3 gözde (%3.6) pupil düzensizliği, 1 gözde (%1.2) ön kamara kaybı meydana geldi. Tashihli görme sonuçları 0.5 ve üstü 74 göz (%88.1), 0.1-0.4 arası 8 göz (%9.5), 0.1'den düşük 2 göz (%2.4) olarak tespit edildi. SONUÇ: Postop hızlı görsel rehabilitasyonu sağlaması, kapalı sistem cerrahi avantajı ile komplikasyon oranının düşük olması, ucuz maliyeti ile mini-nük tekniğinin katarakt cerrahisinde tercih edilmesi gereken bir yöntem olduğu düşüncesindeyiz.
Abstract
PURPOSE: Eightyfour eyes operated on for cataract by mini-nuc technique in Manisa SSK Hospital between January 1999 and March 2000 were evaluated for the intraoperative and postoperative complications and visual results. MATERIAL AND METHODS: Blumenthal's mini-nuc technique was applied under subtenon anesthesia. Scleral tunnel was not sutured. Postoperative results were evaluated for visual scores and complications. RESULTS: The eyes were followed up for a median of 5.5 months (1-12 months). Intraoperatively, posterior capsular rupture in 4 eyes (4.8%), vitreous loss in 2 eyes (2.4 %), iris prolapsus in 2 eyes (2.4%), hemorrhage when replacing IOL in 3 eyes (3.6%), iridodialysis in 2 eyes (2.4%) and perforation of the superior part of the scleral tunnel in 1 eye (1.2%) has occurred. Postoperatively 15 eyes (17.8 %), bullous kerathopathy in 1 eye (1.2%), glaucoma 1 (1.2%), posterior capsule opacification 8 eyes (9.5%,) pupillar irregularity in 3 eyes (3.6%), loss of anterior camera in 1 eye (1.2%) has occurred. Best corrected visual acuity was found 0.5 and better in 74 eyes (88.1%), 0.1- 0.4 in 8 eyes (9.5 %), and under 0.1 in 2 eyes (2.4%). CONCLUSION: Mini-nuc technique is a method to be preferred in cataract surgery, because of early postoperative visual rehabilitation, the advantages of closed system surgery, low amount of complications and lower costs.
