Özet

AMAÇ: Kimyasal korneal yanık nedeniyle penetran keratoplasti yapılan hastaların sonuçlarının değerlendirilmesi YÖNTEM: Penetran keratoplasti yapılan 11 hasta (10 erkek, 1 kadın ) çalışmaya alındı. Ortalama yaş 27.9 (14-55 yaş arası) ve ortalama postoperatif izlem süresi 17.2 ay (6-23 ay arası) idi. BULGULAR: Penetran keratoplasti yapılan 11 hastanın 8'inde (%72.2) greft saydamlığını korudu. Tüm hastalarda preoperatif görme keskinliği 0.1'in altındaydı. Ortalama 17.2 aylık takip -4 sonrasında görme keskinliği 3 hastada 0.1'in altında (%27.2), 5 hastada 0.1-0.4 arası (%45.4) ve 3 hastada 0.5 veya üstü (%27.2) idi. Postoperatif komplikasyonlar 4 hastada sekonder glokom gelişimi (%36), 3 hastada sekonder katarakt gelişimi (%27), 2 hastada bakteriyal keratit (%18) ve bir hastada ise greft rejeksiyonu (%9) şeklinde idi. Üç hastaya penetran keratoplasti öncesi, 2 hastaya ise penetran keratoplasti sonrası limbal kök hücre transplantasyonu uygulandı. SONUÇ: Alıcı korneal yatak yetersizliği, limbal epitelyal kök hücre bozukluğu ve kapak anomalileri kimyasal korneal yanık sonucu yapılan penetran keratoplastilerin prognozunu kötü yönde etkilemektedir. Yapılan limbal kök hücre tranplantasyonu korneal greftin saydamlığını korumasına yardımcı olmaktadır.

Abstract

PURPOSE: The purpose of the study is to determine the outcome of penetrating keratoplasty in chemical corneal burns. METHODS: Records of 11 patients (10 males, 1 female) who underwent penetrating keratoplasty were reviewed. The mean age at surgery was 27.9 (between 14-55) years. The mean follow-up was 17.2 (between 6-23) months. RESULTS: In 8 (%72.2) of 11 patients who underwent primary penetrating keratoplasty, the grafts survived. Preoperative visual acuity was under 0.1 in all patients. After a mean follow-up of 17.2 months, the visual acuity in 3 patients were under 0.1 (%27.2), in 5 patients were between 0.1-0.4 (%45.4), and in 3 patients were above 0.5 (%27.2). The postoperative complications were secondary glaucoma in 4 (%36) patients, cataract in 3 (%27) patients, bacterial keratitis in 2 (%18) patients and graft rejection in 1 patient. Three patients (%27) were underwent limbal transplantation before penetrating keratoplasty and 2 patients after penetrating keratoplasty. CONCLUSION: Insufficiency of the corneal host bed, limbal epithelial stem cell failure and lid abnormalities make the prognosis of penetrating keratoplasty unsatisfactory in chemical corneal burns. Limbal stem cell transplantation is essential for the clarity of the corneal graft.
