Endoskopik intranazal dakriyosistorinostomi sonuçlarımız

Özet

AMAÇ: Primer veya nüks distal nazolakrimal kanal veya kese tıkanıklığı tanısı alan olgularda endoskopik intranazal dakriyosistorinostomi sonuçlarının değerlendirilmesi. GEREÇ VE YÖNTEM: Distal nazolakrimal kanal, ya da kese tıkanıklığı olan olgular 2 grupta incelendi. Birinci grupta 5'i bilateral olmak üzere 17 olgunun toplam 22 gözünde primer obstrüksiyon, ikinci grupta ise 1'i bilateral olmak üzere 4 olgunun 5 gözünde nüks obstrüksiyon mevcuttu. Tüm olguların lakrimal sistem ve intranazal endoskopik cerrahinin uygulanacağı bölgenin anatomik yapısı, operasyon öncesinde lipiodol verilerek koronal planda çekilen bilgisayarlı tomografi ile değerlendirildi. Tüm olgulara endoskopik intranazal dakriyosistorinostomi ameliyatı yapıldı ve aynı seansta silikon tüp ile entübe edildi. Aynı seansta endoskopik olarak cerrahi sahaya engel oluşturabilecek septum deviasyonu ve postoperatif başarıyı etkileyebilecek sinüzit gibi patolojilere müdahale edildi. Silikon tüpler ortalama 6 ay (en az 5 ay-en çok 7 ay) yerinde bırakılıp daha sonra çıkarıldı. BULGULAR: Silikon tüpler çıkarıldıktan sonra birinci grupta bir olguda kanalın tamamen tıkalı olduğu, bir olguda ise parsiyel obstrüksiyon geliştiği gözlendi (başarı oranı %90.91). İkinci grupta tüm olgularda fonksiyonel olarak iyileşme sağlandı (başarı oranı %100). Olgular ortalama 9 ay (en az 6 ay, en çok 14 ay) süreyle takip edildi. SONUÇ: Primer ve nüks distal nazolakrimal kanal veya kese tıkanıklığı olan olgularda, endoskopik yolla dakriyosistorinostomi ameliyatı başarılı olarak bulunmuştur.

Abstract

PURPOSE: We aimed to evaluate the results of endoscopic intranasal dacryocystorhinostomy in patients who had primary or recurrence distal nasolacrimal duct or lacrimal sac obstruction. MATERIALS AND METHODS: The patients with nasolacrimal sac or duct obstruction were divided into two subgroups. In the first group including 17 patients, total of 22 eyes existed with primary obstruction and five of these patients were affected bilaterally; in the second group including 4 patients, 5 eyes existed with recurrent obstruction and one of these patients was affected bilaterally. In all patients lacrimal system and anatomic organization of the surgery were evaluated preoperatively by computerized tomography in the coronal plane performed after lipiodol administration. Endoscopic intranasal dacryocystorhinostomy operation was applied to all cases and silicone tube implantation was performed at the same time. Pathologies that could interfere with the endoscopic manipulation such as septum deviation, or with the postoperative success such as sinusitis, were also treated in the same session. The silicone tubes were removed within 5-7 months. RESULTS: After removal of the silicone tube in the first group, complete duct obstruction was observed in one case and partial obstruction in the second patient (success rate 90.91 %). All cases in the second group were found to be cured functionally (success rate 100%). Mean follow-up period was 9 months (min. 6 months, max.14 months) CONCLUSION: Endoscopic dacryocystorhinostomy operation gives successful results in cases with primary and recurrent distal nasolacrimal duct or lacrimal sac obstruction.
