Özet
AMAÇ: Ambliyopide görme alanı bozukluklarını belirlemek. GEREÇ VE YÖNTEM: Çalışma grubuna toplam 30 ambliyop hasta seçildi. Bu hastaların 10'u anizometropik, 10'u strabismik, 10'u kombine tipte ambliyop idi. Hastaların diğer gözleri, kontrol grubu olarak kabul edildi. Hastalarda, her iki gözde, foveal eşik değeri saptandı ve Humphrey 30-2 full-threshold programı ile otomatik perimetri uygulandı. Humphrey görme alanı STATPAC programının total ve pattern sapma olasılık haritalarına göre, ambliyop gözlerin görme alanları değerlendirildi. Tüm gözlerin foveal eşik, ortalama sapma ve ortalama eşik değerleri incelendi. Görme alanı fiksasyondan itibaren 3, 9, 15, 21 ve 27 derecelik bölümlere ayrılarak, bu bölümlerde ambliyopinin etkisi değerlendirildi. Görme alanının nazal ve temporal yarı alanlarında fark olup olmadığı araştırıldı. Görme keskinliği, görme keskinliğinin logaritması, foveal eşik ve ortalama eşik değerleri arasındaki ilişki incelendi. BULGULAR: Humphrey 30-2 programının olasılık haritalarına göre, 14 ambliyop gözde görme alanı normal olarak değerlendirildi. İki ambliyop gözde santral skotom ve 14 ambliyop gözde jeneralize depresyon saptandı. Ambliyop gözlerde, foveal eşik 5.0±3.8 dB (p 0.05). 30 derecelik görme alanının 0, 3, 9, 15, 21 ve 27 derecelik bölümlerinin tamamında depresyon istatistiksel olarak anlamlı idi (p 0.05). Ambliyop gözde foveal eşik değeri; görme keskinliği (r=.504, p<0.01), görme keskinliğinin logaritması (r=.498) ve ortalama eşik değeri (r=.442, p<0.01) ile korele bulundu. Ambliyop ve diğer göz arasındaki foveal eşik farkı; görme keskinliğinin logaritmik değeri (r=.483) ile korele bulundu. SONUÇ: Ambliyopinin anizometropik, strabismik ve kombine tiplerinde, görme alanı Humphrey 30-2 STATPAC programı ile normal gibi görünmekle birlikte, ışık duyarlılığında foveada en belirgin olmak üzere jeneralize bir depresyon mevcuttur. Ambliyopide, görme alanında fokal ve yarı alan defekti izlenmemiştir.
Abstract
PURPOSE: To define the visual field findings in amblyopia. MATERIAL AND METHODS: Thirty ambliyopia patients have been selected for the study. Ten of these patients have anisometropic ambliyopia, 10 have strabismic ambliyopia and 10 have combined type ambliyopia. Non-ambliyopic eyes of the patients were accepted as the contol group. For all patients, foveal threshold has been measured and automated perimetry using Humprey 30-2 program has been performed. The visual fields of ambliyopic eyes have been evaluated according to the total and pattern deviation plots of Humphrey visual field STATPAC program. Foveal threshold, mean deviation and mean threshold values of all eyes have been measured. The visual field was divided into concentric zones spaced 3 degrees, 9 degrees, 15 degrees, 21 degrees, and 27 degrees from fixation and the effect of ambliyopia on these zones were studied. Hemifield differences in visual fields were searched. Correlations among visual acuity, logarithm of the visual acuity, foveal threshold and mean threshold values were evaluated. RESULTS: According to the total and pattern deviation plots, 14 amblyopic eyes were found normal. Two ambliyopic eyes had central scotoma and 14 ambliyopic eyes had generalized depression. In ambliyopic eyes, foveal threshold [5.0±3.8 dB (p<0.001)], mean deviation [2.7±5.2 dB (p<0.001)] and mean threshold [2.7±2.1 dB (p<0.01)] values were less than the felow eyes. This depression in the light sensitivity of ambliyopic eyes were evident in all three groups of ambliyopia without statistically significant intergroup differences (p>0.05). In ambliyopic eyes, the depression was significant in 3 degrees, 9 degrees, 15 degrees, 21 degrees, and 27 degrees from fixation (p<0.001) and it was most significant at the fovea (p<0.001). In the ambliyopic eye, there was both nasal and temporal depression (p<0.001) but this depression was not hemifield-specific (p>0.05). In the ambliyopic eye, foveal threshold was correlated with visual acuity (r=.504, p<0.01), logarithm of visual acuity (r=.498), and mean threshold (r=.442, p<0.01) values. Foveal threshold differece between the ambliyopic and the fellow eye was correlated with the logarithm of visual acuity (r=.483). CONCLUSION: In anisometricpic, strabismic and combined type ambliyopia, visual field may seem normal with Humphrey 30-2 STATPAC program. However, there is a generalized depression in the visual field, most significant in the fovea. There are no focal or hemifield depressions in ambliyopia.
