Pterjium cerrahisinde basit eksizyon ile basit eksizyon ve ameliyat öncesi subkonjunktival mitomisin-C uygulamalarının karşılaştırılması

AMAÇ: Pterjium cerrahi tedavisi için sadece basit eksizyon ile ameliyat öncesi subkonjunktival mitomisin-C enjeksiyonu ve daha sonra basit eksizyon uygulamalarının nüks oranı ve komplikasyonlar açısından karşılaştırılması amaçlandı. YÖNTEM: 17 hastanın 17 gözüne sadece basit eksizyon yapıldı (Grup l). 19 hastanın 19 gözüne pterjiumun subkonjunktival dokusu içine 0.4 mg/ml konsantrasyonda toplam 0.05 mi mitomisin-C enjekte edildi (Grup II). 2-4 hafta geçtikten sonra bu hastalara basit eksizyon uygulandı. BULGULAR: Ameliyattan 2-4 hafta kadar önce subkonjunktival mitomisin-C enjeksiyonu uygulamasının nüks oranını belirgin olarak azalttığı (%5.8'e karşı %41.1); buna karşılık sübjektif şikayetler ve komplikasyonların arttığı ve bir hastada ciddi sklera incelmesi ve sekonder glokom geliştiği saptandı. SONUÇ: Nüks oranını azaltmak için preoperatif pterjiumun subkonjunktival dokusuna mitomisin-C uygulamasının ciddi komplikasyonlar oluşturabileceği unutulmaması gerekmektedir.
Abstract
PURPOSE: It was aimed to compare the recurrence rates and complications of simple excision versus preoperative subconjunctival mitomycine-C injection and simple excision techniques for the surgical treatment of pterygium. METHODS: Simple excision was performed in 17 eyes of 17 patients (Group I). In 19 eyes of 19 patients 0,05 ml of mitomycin-C solution in a concentration of 0,4 mg/ml was injected into the subconjunctival tissue of pterygium (Group II). Simple excision was then performed in 17 of those 19 subjects after 2 to 4 weeks. RESULTS: Subconjunctival mitomycine-C injection 2-4 weeks prior to surgery was found to reduce the recurrence rate (5.8 versus 41.1%), however subjective complaints and complications were increased and severe scleral thining with secondary glaucoma was developed in one patient. CONCLUSION: Although preoperaitve subconjunctival mitomycin-C injection did reduce the recurrence rate, it should be kept in mind that serious complications could be seen in some patients.
