İskemik ön hazırlamanın sıçan retinasını iskemiye karşı koruyucu etkisi

Özet
AMAÇ: Göz içi basıncı (GİB) değerinin perfüzyon basıncından yüksek değerlere artırılması ile oluşturulan sıçan retinası iskemik modelinde; iskemik ön hazırlamanın (İÖH), iskemik hasara etkisini araştırmak. GEREÇ VE YÖNTEM: Bu çalışmada 150-200 gr arasında 12 adet wistar albino sıçan kullanıldı. Retinal iskemi, yüksek göz içi basınç yöntemi ile oluşturuldu. İki gruba ayrılan sıçanlardan birinci grubun sağ gözlerine beş dakika boyunca yüksek basınç uygulanarak İÖH yapıldı, diğer grubun sağ gözlerine aynı koşullar altında, basınç uygulanmadan yalancı işlem (Yİ) yapıldı. Her iki gruba 24 saat sonra 60 dakika boyunca retinal iskemi oluşturuldu. İskemik işlemden hemen sonra retinalar histolojik inceleme için takibe alındı. BULGULAR: Yİ yapılan grubun toplam retina kalınlığı İÖH yapılan gruba göre anlamlı düzeyde incelmişti (p 0.05). Fotoreseptör tabakası (FRT) kalınlığı Yİ grubunda İÖH grubuna göre anlamlı düzeyde daha fazlaydı (p<0.001). İÖH grubunda ganglion hücre tabakası (GHT), iç pleksiform tabaka (İPT) ve iç nukleer tabakadan (İNT) oluşan iç tabakalar toplam retina kalınlığının %47.5±1.3'ini oluştururken aynı tabakalar Yİ grubu toplam retina kalınlığının %41.6±1.8'sını oluşturuyordu. İÖH grubunda, DPT, dış nukleer tabaka (DNT) ve FRT'dan oluşan dış tabakalar toplam retina kalınlığının % 52.5±1.3'ini oluştururken, aynı tabakalar Yİ grubunun %58.4±1.8'sini oluşturuyordu. Her iki grup için iç ve dış tabakaların toplam retinaya göre oranları arasında anlamlı fark vardı. SONUÇ: İÖH retinayı daha sonraki iskemiye karşı korumaktadır ve bu koruyucu etki iç retinal tabakalarda daha belirgindir.
Abstract
PURPOSE: To assess the protective effect of ischemic preconditioning (IP) in ischemic rat retina model. MATERIAL AND METHODS: In this study, 12 albino wistar rats weighing 150-200 gr were used. The retinal ischemia was obtained with high IOP. Rats were divided in two groups each consisting of 6 animals. The retinas of the first group of rats were preconditioned with 5 minutes of ischemia; ischemic preconditioning (IP), second group of rats were sham operated under the same conditions. The retina of both group of rats were rendered ischemic for 60 min after 24 hours following the first procedure. After the second ischemic period the retina of the sacrified rats were processed for histological analysis. RESULTS: The total retinal thickness of the sham group was significantly thinner than the IP group (p<0.001). All of the retinal layers of the IP group except outer plexiform layer (OPL) and photoreceptor cell layer (PCL) were significantly thicker than the sham group (p<0.001). PCL of the sham group was significantly thicker than the IP group (p<0.001). Inner layers consisting of ganglion cell layer (GCL), inner plexiform layer, and inner nuclear layer (INL) made up 47.5±1.3 % of the total retinal thickness in the IP group whereas same layers made up the 41.6±1.8 % of the retinal thickness of the sham group. Outer layers consisting of OPL, outer nuclear layer (ONL), and PCL made up 52.5±1.3 % of the total retinal thickness of the IP group whereas same layers made up 58.4±1.8 % of total retinal thickness in the sham group. CONCLUSION: IP protected retina from the effects of subsequent ischemia in a high IOP ischemic rat retina model.
