Keratokonüsde kornea ve gözyaşı değişiklikleri

Özet
AMAÇ: Keratonusun kornea epitel ve stromasında artmış apoptozise ve morfolojik değişikliklere yol açabileceği daha önce gösterilmiştir. Bu değişikliklerin kornea ve gözyaşı fonksiyonlarını nasıl etkilediği henüz tam olarak bilinmemektedir. Bu prospektif çalışmada keratokonüs ve değişik evrelerinin kornea hassasiyeti ile gözyaşı fonksiyonlarını nasıl etkilediği araştırılmıştır. GEREÇ VE YÖNTEM: Topografi, pakimetri ve biyomikroskopi ile keratokonüs teşhisi konmuş 14 olgunun 28 gözü ile, kontrol grubu olarak 15 olgunun 30 gözüne kornea hassasiyet ölçümleri, Schirmer testi, floresein parçalanma zamanı (FPZ), floresein ve Rose-Bengal boya skorlaması yapılmıştır. BULGULAR: Keratokonuslu olguların biyomikroskopi muayenesinde sıklıkla yüzeyel noktalı keratit ve ön stromal retiküler skarlaşma gibi korneal sorunlara rastlanmıştır. Keratokonüslü olgularda kontrol olgularına kıyasla göz yaşı kırılma zamanı ve kornea hassasiyetinde anlamlı azalma, floresein ve Rose Bengal skorlarında anlamlı artış bulunmuştur. Bu parametrelerin hastalığın ilerlemiş evrelerinde anlamlı olarak bozulma gösterdiği de saptanmıştır. SONUÇ: Keratokonüste hastalığın ilerlemesi ile beraber gözyaşı kalitesi ve kornea sağlığında bozulma görülmüştür. Keratokonus etyopatogenezinde kornea epitelinin primer ya da sekonder önemli bir rol oynayabileceği düşünülmüştür.
Abstract
PURPOSE: It has previously been shown that keratoconus might lead to increased apoptosis and morphological alterations in the corneal epithelium and stroma. How these alterations effect the cornea and tear function are still not well known. In this prospective study, how keratoconus and different stages of the disease process effect the corneal sensitivity and tear functions have been investigated. MATERIAL AND METHODS: Twenty-eight eyes of 14 cases diagnosed as keratoconus with the aid of topography, pachymetry and biomicroscopy and 30 eyes of 15 control subjects underwent corneal sensitivity measurements, Schirmer test, tear film break up time, fluorescein and Rose-Bengal score assessment. RESULTS: Biomicroscopy examination revealed frequent corneal problems like superficial punctate keratopathy and anterior stromal reticular scarring in keratoconus. Tear film break up time and corneal sensitivity revealed a significant decrease whereas fluorescein and Rose-Bengal scores showed a significant increase in keratoconus patients when compared to controls. These parameters have been detected to deteriorate with advanced stages of disease. CONCLUSION: Tear film quality and corneal health were seen to deteriorate in close proximity to the progression of the keratoconus disease process.
