Pterjiumun cerrahi tedavisinde kullanılan mitomisin-C'nin göziçi basıncına etkisi

Özet
AMAÇ: Pterjiumun cerrahi tedavisinde nüksleri önlemek amacıyla kullanılan mitomisin-C'nin (MMC) farklı konsantrasyonlarda episkleral uygulanımının göz içi basıncı (GİB) üzerine etkisini incelemek. GEREÇ VE YÖNTEM: Aralık 1999- Mayıs 2000 tarihleri arasında, Ankara Numune Hastanesi 1. Göz Kliniğine pterjium nedeniyle başvuran hastalardan cerrahi tedaviye karar verilenler onar hastadan oluşan üç gruba ayrılarak nüksü önleme amacıyla ekstirpasyonu takiben; 1.gruba 0.1 mg/ml, 2. gruba 0.2 mg/ml ve 3. gruba 0.5 mg/ml konsantrasyonlarda MMC beşer dakika süreyle episkleral olarak uygulandı. Hastaların her üç grupta preoperatif ile postoperatif 3.gün, 1.hafta, 2. hafta, 1.ay ve 2.aylardaki GİB'ları karşılaştırıldı. BULGULAR: MMC'nin 0.5 mg/ml konsantrasyonda kullanıldığı 3. gruptaki hastalarda postoperatif dönemde GİB'nda istatistiksel olarak anlamlı düşme saptanırken 0.1 mg/ml ve 0.2 mg/ml gruplarında anlamlı değişim izlenmedi. SONUÇ: Filtran cerrahiler sırasında episkleral fibrozise bağlı olarak ortaya çıkan başarısızlığı önleme amacıyla kullanılan MMC'nin filtran girişimler dışındaki uygulamalarda da konsantrasyon ile bağlantılı olarak silier cisim üzerindeki muhtemel toksik etkisi ile GİB düşüşüne neden olabileceğine dikkat çekildi.
Abstract
PURPOSE: To investigate the effect of mitomycin-C (MMC) on the intraocular pressure (IOP) when used as an adjunctive therapy in the surgical treatment of pterygium to prevent recurrences. MATERIAL AND METHOD: In Ankara Numune Hospital Eye Clinic, between December 1999 and May 2000, thirty patients who were diagnosed as pterygium and were candidates for surgical treatment were divided to three groups each consisting of 10 patients. Episcleral applications of MMC were performed for 5 minutes during pterygium extirpation in order to prevent the recurrences with a concentration of 0.1 mg/ml in Group 1, 0.2 mg/ml in Group 2 and, 0.5 mg/ml in Group 3. Intraocular pressure (IOP) levels were measured preoperatively, then in the 3rd day, 1st week, 2nd week, 1st month, 2nd month postoperatively and compared statistically. RESULTS: IOP levels were significantly decreased postoperatively in Group 3 in which 0.5 mg/ml MMC had been used, however no significant differences were noted in 0.1 mg/ml and 0.2 mg/ml groups. CONCLUSION: MMC which is used adjunctively in glaucoma filtration procedures in order to prevent bleb failures due to episcleral fibrosis might also show pressure lowering effects in ophthalmic procedures other than filtration surgery possibly by its toxicity on the ciliary body.
