Normotansif glokomlu olgularda noktürnal hipotansiyon

Özet
AMAÇ: Normotansif glokomlu (NTG) olgularda noktürnal hipotansiyon ile glokomatöz optik nöropatinin progresyonu arasındaki ilişkinin araştırılması. GEREÇ VE YÖNTEM: Ankara Numune Eğitim ve Araştırma Hastanesi 1. Göz Kliniği Glokom Biriminde NTG tanısıyla izlenen hastalardan 32 olgunun görme alanı (GA) hasarı daha ileri olan 32 gözü çalışma kapsamına alındı. Seçilen gözlerin birer yıl aralıklı ikişer GA testi Advanced Glaucoma Intervention Study (AGIS) derecelendirme yöntemi ile glokomun ilerlemesi açısından değerlendirildi. Olguların tümüne 24 saatlik ambulatuar kan basıncı takibi yapıldı. Glokomatöz ilerleme gösteren ve göstermeyen olgular; yaş, göziçi basınçları (GİB), çukurluk/disk (C/D) oranları, sistemik antihipertansif ilaç kullanımları, sistolik ve diastolik kan basınçları ile noktürnal çökme (dip) değerleri açısından karşılaştırıldı. BULGULAR: Olguların 18'inde (%56.2) AGIS kriterlerine göre glokomatöz ilerleme mevcuttu. İlerleme izlenen ve izlenmeyen iki grup arasında yaş, C/D oranları ve GİB'ları açısından fark saptanmadı. Antihipertansif tedavi kullanma oranı progresif grupta %50.0 iken stabil grupta %28.6 idi. Gruplar arasında gündüz-gece sistolik ve diastolik kan basınçları açısından farklılık izlenmemekle birlikte progresif grupta noktürnal sistolik ve diastolik diplerin ve dip saptanan olguların (dipper) oranları istatistiksel olarak anlamlı düzeyde yüksek bulundu (tümü için; p<0.05). SONUÇ: Tedaviye rağmen ilerleme gösteren NTG'lu olgularda 24 saatlik ambulatuar kan basıncı monitörizasyonunun noktürnal dipleri ortaya çıkararak, progresyonun mekanizması hakkında bilgi verebileceği ve esansiyel hipertansiyonlu olgularda antihipertansif tedavinin buna göre yeniden düzenlenmesinin yararlı olabileceği sonucuna varıldı.
Abstract
PURPOSE: To investigate the relation between nocturnal hypotension and progression of glaucomatous optic neuropathy in normal tension glaucoma. MATERIALS AND METHODS: Thirty-two normal tension glaucoma (NTG) patients who were being followed-up in the Glaucoma Department of Ankara Numune Education and Training Hospital 1st Eye Clinic were enrolled in the study. Two one-year-apart visual field (VF) tests were evaluated for progression according to the criteria of the Advanced Glaucoma Intervention Study (AGIS) in the eye with more damaged VF in each patient. Twenty-four-hour ambulatory blood pressure measurements was performed in each case. Patients with or without glaucomatous progression were compared in terms of patient age, intraocular pressure (IOP), cup/disc (C/D) ratios, systemic antihypertensive drug use, systolic and diastolic pressures and nocturnal dips. RESULTS: Glaucomatous progression was present in 18 patients (56.2%) according to the AGIS criteria. No significant difference was found in terms of age, IOP and C/D ratios between the groups with and without glaucomatous progression. Systemic antihypertensive drug use was 50.0% in the progressive and 28.6% in the stable group. Day and night, systolic and diastolic blood pressures were similar in the two groups, however, systolic and diastolic nocturnal dips and the ratio of dippers were significantly higher in the group of progressive cases (p<0.05, for each item). CONCLUSION: Twenty-four-hour ambulatory blood pressure monitoring might detect nocturnal dips and disclose the mechanism of progression in cases with NTG which are resistant to treatment. Reestablishment of antihypertensive therapy according to this data in cases with coexisting essential hypertension might be beneficial.
