Ayarlanabilir sütür tekniğinde ayarlama insidansı ve etkinliği

Özet
AMAÇ. Ayarlanabilir sütür tekniği uygulanan şaşılık olgularında, postoperatif sütür ayarlama oranının, olguların özellikleri ile olan bağlantısının incelenmesi. GEREÇ VE YÖNTEM: Onaltı ezotropya (ET), 32 ekzotropya (XT) ve 6 hipertropya (HT) olgusu çalışma kapsamına alındı. Postoperatif 1.gün gereken olgulara topikal anestezi altında ayarlama uygulandı. Olgular en az 6 ay süreyle takip edildi. BULGULAR: Horizontal kaymalı 25 olguya (%52) ayarlama yapılırken, 23'üne (%47.9) ayarlama gerekmedi. Bu olguların kaymaları ortalama preoperatif 19.5 PD, postoperatif 3.6 PD idi. ET olgularının 5'ine (%31.25) ayarlama yapılırken (Grup 1), 11'ine (%68.8) gerekmedi (Grup 2). Grup 1'de ortalama kayma preoperatif 34 PD, postoperatif 5.6 PD olup Grup 2'de bu oran 41.8 PD ve 5.7 PD idi. XT olgularının 20'sine (%62.5) ayarlama yapılıp (Grup 3), 12'sine (%37.5) yapılmadı (Grup 4). Grup 3 de kayma preoperatif 37.25 PD, postoperatif 8.6 PD, Grup 4'de preoperatif 37.91 PD, postoperatif 6.1 PD idi. Gruplar arasında preoperatif ve postoperatif deviasyonlar arasında fark yoktu. Hipertropyalı 4 olguya (%66.7) ayarlama yapıldı (Grup 5), ikisine yapılmadı (Grup 6) (%33.3). Ayarlama yapılan HT'li olguların (Grup 5) ortalama kaymaları preoperatif 19.5 PD postoperatif 3.6 PD idi. Ayarlama yapılmayan (Grup 6) HT'li olguların preoperatif kaymaları ortalama 20PD, postoperatif dönemde ise 4 PD idi. Hipertropyalı olguların preoperatif ve postoperatif kaymaları horizontal kaymalı olgulara göre düşük olup, bu olgulardan ayarlama yapılan ve yapılmayanlar arasında fark yoktu. SONUÇLAR: Postoperatif sütür ayarlama oranının XT ve HT olgularında yüksek olduğu postoperatif kayma derecesinin ayarlama yapılan ve yapılmayan olgularda farklı olmadığı gözlendi. Bu teknikle ameliyat edilen olgulardaki sütür ayarlama insidansının yüksek olması, endike olgularda postoperatif başarıyı arttırmak için ayarlanabilir sütür tekniğinin etkili ve gerekli bir yöntem olduğu görüşünü desteklemektedir.
Abstract
PURPOSE: To evaluate the need of postoperative adjustment in adjustable suture in squint cases according to their clinics. MATERIAL AND METHODS: The study population includes 16 esotropia (ET), 32 exotropia (XT) and 6 hypertropia (HT) cases who underwent strabismus surgery with adjustable suture technique. Postoperative 1st day adjustment was performed in the cases in need of it. RESULTS. Of 25 cases (52%) with horizontal deviation were in need of suture adjustment. Preoperative and postoperative deviations of those cases were 19.5PD and 3.6 PD, respectively. Five cases (31.25%) with esotropia were in need of adjustment (Group 1) and 11 (68.8%) were not (Group 2). Twenty cases (62.5%) with exotropia were in need of adjustment (Group 3) and 12 (37.5%) were not (Group 4). Preoperative and postoperative deviations in Group 3 were 37.25PD and 8.6 PD, respectively and in Group 4 those deviations were 37.91 PD and 6.1 PD. There were no statistically significant difference between the preoperative and postoperative deviations in groups. Four cases (66.7%) with hypertropia underwent suture adjustment (Group 5) and 2 cases were not required adjustment (Group 6) Preoperative and postoperative deviations of cases in Group 5 were 19.5 PD and 3.6 PD respectively and those deviations were 20 PD and 4 PD in Group 6. Vertical deviations in both groups were less than that of patients with horizontal deviations and there were no significant difference between the average preoperative and postoperative deviations in Group 5 and 6. CONCLUSION: The incidence of suture adjustment was higher in cases with XT and HT. Postoperative deviation did not differ between cases whose sutures were adjusted and cases were not in need of adjustment. The high incidence of adjustment in adjustable suture technique indicates that adjustable suture technique is necessary and an effective method.
