Yüksek riskli kornea transplantasyonunda HLA-DR antijen uygunluğunun greft reddine etkisi

AMAÇ: Yüksek riskli kornea transplantasyonunda HLA-DR uygunluğunun immünolojik grefon reddi üstüne etkisini araştırmak. GEREÇ VE YÖNTEM: Bu çalışma yüksek riskli parsiyel penetran keratoplasti ameliyatı geçiren 47 hastanın 47 gözünü içerdi. HLA-DR antijenlerini tiplendirmek için "polimerase chain reaction" ve "reverse dot blot hybridization" yöntemleri kullanıldı. Alıcı ile verici arasında, HLA-DR antijenleri açısından 18 hastada tam uygunluk vardı, 29 hastada ise tek allel uygunluğu vardı veya hiç uygunluk yoktu. Hasta grupları, postoperatif 18 ay içinde gelişen korneal greft reddi açısından karşılaştırıldı. BULGULAR: Tam HLA-DR uyumlu grupta hastaların %22'sinde (4/18 hasta), düşük veya sıfır uyumlu grupta ise hastaların %14'ünde (4/29 hasta) dönüşümsüz greft reddi gelişti. İki grup arasında red oranı ve greft ömrü açısından anlamlı bir farklılık yoktu. (P=0.34 ve P=0.69). SONUÇ: Bu çalışmada, yüksek riskli kornea transplantasyonunda HLA-DR uygunluğunun, greft reddini azaltmakta yararlı bir etkisi yoktu. İstatistiksel olarak anlamlı olmamakla birlikte, korneal greft reddi, tam HLA-DR uygunluğu olan gözlerde daha sıktı.
Abstract
PURPOSE: We investigated the effect of HLA-DR matching on immunological graft rejection in high-risk corneal transplantation. MATERIAL AND METHOD: This study included 47 eyes of 47 patients who underwent high-risk partial penetrating keratoplasty. The polimerase chain reaction and reverse dot-blot hybridization methods were used for typing HLA-DR antigens. Between the donors and recipients of HLA-DR antigens, there was exact matching in 18 patients (Group I), and low or no matching in 29 patients (Group II). The patient groups were compared for corneal graft rejection which developed within 18 months postoperatively. RESULTS: Irreversible graft rejection developed in 22% of patients in the exact matched group (4/18 patients) and in 14% of patients in the low or zero matched group. There were no differences between the two groups for the rate of rejection and graft survival (P=0.34 and P=0.69). CONCLUSION: In this study, HLA-DR matching in high-risk corneal transplantation had no beneficial effect in reducing graft rejection. Although not statistically significant, corneal graft rejection was more frequent in eyes with good HLA-DR matching.
