İskemik santral retina ven oklüzyonlarının değerlendirilmesi

Özet
AMAÇ: İskemik santral retina ven oklüzyonlarında risk faktörleri, kliniği, komplikasyonları ve prognozu irdelemek. GEREÇ VE YÖNTEM: İskemik santral retina ven oklüzyonu olan hastalar yaş, cins, takip süreleri, sistemik hipertansiyon ve diyabetin varlığı, diğer gözün durumu ve komplikasyonlar yönünden incelendi. İlk ve son görme düzeyleri gözden geçirilip , istatistiksel olarak karşılaştırıldı. BULGULAR: Çalışma kapsamına 35 olgunun, 35 iskemik santral retina ven oklüzyonlu gözü alınmıştır. Ortalama yaşları 59.8±11.2 yıl (39-80 yıl ) olan 19 erkek (% 54.3), 16 kadın (% 45.7) hasta mevcuttu. Ortalama takip süreleri 11.7± 6.3 aydı (2-98 ay). Hastalarımızda % 65.7 oranında sistemik hipertansiyon, % 20 oranında diyabetes mellitus birlikteliği saptandı. Diğer gözde hipertansif retinopatinin % 51.4 oranı ile en sık patoloji olduğu gözlendi. Olguların başlangıç görme keskinlikleri ışık persepsiyonu ve projeksiyonu ile 0.4 (ort: 0.056±0.008) düzeyleri arasında, final görme keskinlikleri ise absolu ile 0.3 (ort: 0.025±0.002) düzeyleri arasında idi. Aradaki farkın istatistiksel olarak anlamlı olduğu saptandı (p<0.05). Komplikasyonlar olarak; 2 hastada (%5.7) rubeozis iridis, 2 hastada (%5.7) neovasküler glokom, 2 hastada (%5.7) vitreus hemorajisi, 4 hastada (%11.4) retina neovaskülarizasyonu, 6 hastada (%17.1) ise disk neovaskülarizasyonu gelişti. Tüm gözlerde maküla ödemi olduğu saptandı. SONUÇ: İskemik santral retina ven oklüzyonlarında sistemik hipertansiyonun önemli bir risk faktörü olduğu, patolojinin görmeyi tehdit eden komplikasyonları olup görsel prognozunun iyi olmadığı düşünülmüştür.
Abstract
PURPOSE: To evaluate the risk factors, clinics, complications and prognosis of ischemic central retinal venous occlusion. MATERIAL AND METHODS: Patients with ischemic central retinal venous occlusion were evaluated for age, gender, follow-up period, systemic hypertension, diabetes mellitus, state of the other eye and complications. Their baseline and final visual acuities were looked over and compared statistically. RESULTS: Thirty five eyes of 35 patients whom were followed for ischamic central retinal venous occlusion were included in the study. There were 19 males (54.3%) and 16 females (45.7%) with a mean age of 59.8±11.2 years (39-80 yrs.). The mean follow-up period was 11.7±6.3 months (2-98ms). Systemic hypertension and diabetes mellitus were observed with the rates of 65.7 % and 20 % respectively. The most frequent pathology of the other eye was hypertensive retinopaty with a rate of 51.4 %. The baseline visual acuities were between light perception and projection and 0.4 (mean: 0.056±0.008) while the final visual acuities were between absolute and 0.3 (mean:0.025±0.002). The difference was statistically significant (p<0.05). Rubeosis iridis in 2 eyes (5.7%9), neovascular glaucoma in 2 eyes (5.7%), vitreus hemorrhage in 2 eyes (5.7%), neovascularization of retina in 4 eyes (11.4%) and neovascularization of disk in 6 eyes (17.1%) were observed as the complications. Macular edema was diagnosed in all eyes. CONCLUSION: Ischemic central retinal venous occlusion which has complications threatining the sight does not have a good visual prognosis and systemic hypertension is one of the important risk factors of the disease.
