Perforan göz yaralanmalarında erken müdahalenin görme prognozu üzerine etkisi

Özet
AMAÇ: Perforan göz yaralanmalarında müdahale zamanı ile görme prognozu arasındaki ilişkiyi araştırmak. GEREÇ VE YÖNTEM: Ekim 1997-Mart 2001 tarihleri arasında, perforan göz yaralanması ile kliniğimize başvuran 258 olgunun 275 gözü, travma olduktan sonra ameliyata alınana kadar geçen süre açısından iki gruba ayrıldı. Yirmidört saat ve daha erken müdahale edilen 207 olgunun 220 gözü erken müdahale grubu (1. grup), ilk 24 saatten sonra müdahale edilen 51 olgunun 55 gözü geç müdahale grubu (2. grup) olarak adlandırıldı. Olguların, ilk muayenedeki ve son kontrolleri sırasındaki düzeltilmiş görme keskinlikleri değerlendirmeye alındı. Görme keskinliği dereceleri parmak sayma (PS) ve daha düşük (kötü), 0.1-0.3 arası (orta) ve 0.4-1.0 arası (iyi) olarak üç gruba ayrıldı. Olgular ortalama 8.25±2.3 (5-18) ay takip edildi. İstatistiksel değerlendirmeler bağımsız t-testi ve ki-kare testi ile yapıldı. BULGULAR: İlk görme keskinliği kötü düzeyde olan göz sayısı 1. grupta 105 (%47.72), 2. grupta 39'du (%70.90). İlk görmesi iyi düzeyde olan göz sayısı ise 1. grupta 94 (%42.73), 2. grupta 10 (%18.19) olarak bulundu (p<0.01). Son görme keskinliği kötü düzeyde olan göz sayısı 1. grupta 58 (%26.36), 2. grupta ise 31 (%56.36) olarak saptandı (p<0.01). Son görmesi iyi düzeyde olanların sayısı ise 1. grupta 148 (%67.27), 2. grupta 20 (%36.37) olarak bulundu (p<0.01). İkinci gruptaki olguların ilk muayenesinde dört gözde endoftalmi, iki gözde travmatik üveit ve iki gözde rubeozis iridis saptandı. SONUÇ: Perforan göz yaralanmalarında cerrahi müdahalenin gecikmesi, görme prognozunu olumsuz etkilemekte ve komplikasyon riskini artırmaktadır.
Abstract
PURPOSE: To investigate the relationship of the operation timing and the visual prognosis in perforating eye injuries. MATERIAL AND METHODS: Two hundred and seventy five eyes of 258 patients with perforating eye injuries applied to our clinic between October 1997-March 2001 were divided into two groups according to the interval between trauma and surgery. Two hundred and twenty eyes of 207 cases who were operated within 24 hours after the trauma were appointed as early intervention group (Group 1), and 55 eyes of 51 cases who were operated later than 24 hours after the trauma as late intervention group (Group 2). Corrected visual acuities of the cases at the initial examination and last visits were evaluated. Visual acuities were divided into three categories according to finger counting or worse (poor), between 0.1 and 0.3 (moderate), and 0.5 or better (good). The cases were followed-up for a mean of 8.25±2.3 (5-18) months. Statistical evaluations were performed with independent-samples t-test and chi-square test. RESULTS: The number of eyes with poor initial visual acuity were 105 (47.72%) in Group 1 and 39 (70.90%) in Group 2 (p<0.001). The number of eyes with good initial visual acuity were 94 (42.79%) in Group 1 and 10 (18.19%) in Group 2 (p<0.001). The final visual acuity were poor in 58 eyes (26.36%) in Group 1 and in 31 eyes (56.36%) in Group 2 (p<0.001). The final visual acuities were good in 148 patients (67.27%) in Group 1 and in 20 eyes (36,37%) in Group 2 (p<0,001). At the initial examination in Group 2, endophthalmitis was found in four eyes and traumatic uveitis and rubeosis iridis in two eyes. CONCLUSION: The late surgical intervention of the perforating eye injuries has negative effect on the visual prognosis and high complication risk.
