Uzun süre latanoprost ile tedavi edilen primer açık açılı glokomlu olgularda renkli doppler ultrasonografi ve otomatik perimetrideki değişimler

Özet
AMAÇ: Primer açık açılı glokomlu hastalarda oküler kan akımı ve görme alanı üzerine %0.005 latanoprostun uzun dönem etkisini değerlendirmek. GEREÇ VE YÖNTEM: Çalışma 22 hastanın 22 gözünü kapsadı. Her bir göze %0.005 latanoprost akşam saat 21:00 da 1 kez damlatıldı. Tüm gözlere tedaviden hemen önce, 6 ve 12 ay sonra renkli Doppler ultrasonografi (RDU) ve görme alanı (Humphrey görme alanı analizörü-santral 30-2 tam eşik) testi yapıldı. BULGULAR: Tedavi öncesine göre, latanoprost 6 ve 12. ayda istatistiksel olarak anlamlı göz içi basınç (GİB) düşüklüğüne neden oldu (P<0.01). Tedavi öncesi ortalama GİB'dan ortalama yüzde azalması 6. ayda %32.57, 12. ayda %34.84'dü. Santral retinal arterin tedavi öncesine gore, 6 ve 12. ayda ortalama rezistivite indeksi (Rİ) istatistiksel olarak anlamlı düşüktü (sırasıyla p<0.001, p<0.01). Latanoprost tedavisinden sonra ortalama sapma, tedavi öncesine göre istatistiksel olarak anlamlı düşme gösterdi (p<0.001). SONUÇ: Latanoprost 12 aylık tedavi sırasında GİB'da anlamlı ve stabil bir düşmeye neden olup, santral retinal arter Rİ'de azalmaya yol açmıştır.
Abstract
PURPOSE: To evaluate the longterm effects of 0.005% latanoprost on ocular blood flow and visual field in patients with primary open angle glaucoma (POAG). MATERIAL AND METHODS: This study consisted of 22 eyes of 22 patients with POAG. Each eye received one drop of 0.005% latanoprost at 21:00 pm. All eyes underwent color Doppler ultrasonography (CDU) and visual fields examination (Humphrey visual field analyzer-central 30-2 full threshold test) before, 6 and 12 months after dosing. RESULTS: Latanoprost caused a statistically significant reduction in intraocular pressure (IOP) from baseline at 6 and 12 months (P<0.01). The average percent decrease from baseline mean IOP was 32.57% at 6 months and 34.84% at 12 months. The mean resistivity index (RI) of the central retinal artery (CRA) was statistically significantly lower at 6 and 12 months of treatment than baseline (p<0.001, p<0.01 respectively). The average mean deviation showed statistically significant decrease after latanoprost treatment than baseline (p<0.001). CONCLUSION: Latanoprost causes a significant and stable IOP reduction with altering ocular blood flow in CRA during 12 months of therapy.
