Özet
Amaç: Pars plana fakofragmantasyon yöntemi ile vitreustaki lens parçalarının çıkarıldığı olgularda, cerrahi ve görsel sonuçların değerlendirilmesi. Gereç ve Yöntem: Fakoemülsifikasyon komplikasyonu olarak, vitreusa lens parçalarının düştüğü sekiz olgunun sekiz gözü çalışma kapsamına alındı. Tüm olgularda cerrahi teknik, görsel ve cerrahi sonuçlar değerlendirildi. Bulgular: Hastaların yaş ortalaması 69.2±5.1 (61-80), ortalama izlem süresi ise 8.1±2.2 aydı (5-12). Preoperatif düzeltilmiş en iyi görme keskinliği el hareketi ile 0.4 arasında değişmekteydi. Olguların tümü afak olup, ameliyata kadar geçen süre ortalama 2.4&2.1 haftaydı (1-5 hafta). Tüm gözlerde pars plana vitrektomi ile kombine pars plana fakofragmantasyon uygulandı. Ön kapsül desteğinin yeterli olduğu altı göze küçük keşiden sulkusa katlanabilir akrilik göziçi lens (GİL), ön kapsül desteğinin yeterli olmadığı iki göze de skierai fiksasyonlu katlanabilir akrilik GİL implantasyonu yapıldı. Postoperatif ortalama görme keskinliği 0.5±0.2 (0.1-0.7) olarak saptandı. İzlemde GİL'in sulkusa yerleştirildiği bir olguda, GİL postoperatif birinci ayda desantralize oldu. Bir gözde de, postoperatif dördüncü ayda yırtıldı retina dekolmanı gelişti. Sonuç: Lens parçalarının vitreustan çıkarılmasında pars plana fakofragmantasyonun alternatif bir yöntem olabileceği düşünüldü.
Abstract
Purpose: To evaluate the surgical and visual outcome of the removal of lens fragments from the vitreous with pars plana phacofragmantation. Material and Methods: Eight eyes of eight patients with lens fragments in the vitreous as a complication of phacoemusiflcation surgery were included in the study. In all cases surgical technique, surgical and visual outcomes were evaluated. Results: The mean age of the patients was 69.2±5.1 years (61-80), and the mean follow-up was 8.1 ±2.2 months (5-12). The preoperative best corrected visual acuity was between hand motion and 0.4. All cases were aphakic. Time elapse between two surgeries were 2.4±2.1 weeks (1-5 weeks). In all eyes, pars plana vitrectomy combined with pars plana phacofragmantation was performed. In six eyes in which the anterior capsular rim was intact, sulcus fixated foldable hydrophylic acrylic lOLs were implanted whereas in 2 eyes in which there was no adequate capsular rim transsclerally sutured foldable acrylic lOLs were implanted. The postoperative mean visual acuity was 0.5±0.2 (0.1-0.7). In the follow-up, in one eye, sulcus fixated IOL had been decentred in postoperative first month, and in one eye a rhegmatogenous retinal detachment was detected in the postoperative fourth month. Conclusion: Pars plana phacofragmantation can be an alternative technique for the removal of lens fragments from the vitreous.
