Çocuklarda travmatik katarakt cerrahisi sonrası oluşan fibrinoid reaksiyonu önlemede preoperatif kortikosteroidin önemi

Özet
Amaç: Çocuklarda travmaya bağlı katarakt cerrahisi sonrası fibrinoid reaksiyonu önlemede preoperatiftek doz sistemik kortikosteroidin (KS) etkisini incelemek. Gereç ve Yöntem: Yaşları 14'ten küçük olan ve 45 'inde kunt travma, 28 'inde korneal perforasyon nedeniyle katarakt gelişen ve ameliyat edilen 73 çocuk çalışma kapsamına alındı. Her iki gruptan 38. hastaya ameliyat öncesi KS verilmezken, kalan 35 olguya ise l mg/kg tek doz IV prednizolon verilerek ameliyat edildi. Bütün hastalarda ameliyat sonrası erken dönemdeki fibrinoid reaksiyon durumu değerlendirildi. Bulgular: Kunt travma geçiren 26 çocuktan (A grubu, KS verilmeyen) 5 'inde (%19.2) ciddi olmak üzere toplam 12'sinde (%46.1) , B grubundaki (kunt travma ve KS verilen) 19 çocuktan 1 tanesinde (%5.2) ciddi olmak üzere toplam 5 'inde (%26.3) fibrinoid reaksiyon gelişti. İki grup arasındaki farklar istatistiksel olarak anlamlı idi (p<0.05). Korneal perforasyon geçiren ve kortikosteroid verilmeyen C grubundaki 12 hastanın 7'sinde (%58.3), buna karşılık KS verilen 16 hastanın (D grubu) 5'inde (%31,2) yoğun fibrihpid reaksiyon gelişti. C ve D grubundaki yoğun reaksiyon görülen hasta oranları arasındaki fark istatistiksel olarak anlamlıydı (p<0.05). Sonuç: Çocuklarda gerek kunt ve gerekse delici yaralanma sonucu gelişen karatarklarda cenahı .sonrası oluşabilecek fibrinoid reaksiyonu azaltmak amacıyla preoperatif tek doz kortikosteroid kullanılmasının uygun olacağı kanaatindeyiz.
Abstract
Purpose: To investigate the effect of single dose corticosteroid (CS) in preventing the fibrinous reaction after cataract surgery due to trauma in children. Material and Methods: The study consists of seventy-three children that vere younger than 14 years of age, and had secondary cataract due to trauma. The patients were divided into two groups as blunt trauma (45 cases) and cornea! perforation (28 cases). Prednisolone (lmg/kg) was given to 35 Children preoperatively, was not given to the remaining 38 children. After cataract surgery, anterior chamber reaction was evaluated in all children. Result: Fibrinous reaction was observed in 12 (46.1%) of 26 children in group A (blunt trauma and no CS) and 5 (19.2%) of them were severe, while observed in 5 (26.3%) of 19 children in group B (blunt trauma and given CS) and one (5.2%) of them was severe. The differences between two groups was statistically significant (p<0.05). Fibrinous reaction was developed in 7 (58.3%) of 12, children in group C (perforation and no CS), and in 5 (31.2%) of 16 children in group D (perforation and given CS). The differences between group C and D were also statistically significant (p<0,.0 5). Conclusion: The rate of fibrinous reaction in anterior chamber may decrease with preoperative corticosteroid treatment in children with cataract due to blunt or perforating trauma.
