Mini-nük tekniğinde cerrahiye bağlı astigmatizmanın kontrolü

Özet
Amaç: Mini-nük tekniğindeki sklerokorneal tünel yapılanmasında 5.5-6.5 mm'lik dış kesinin yerini değiştirerek cerrahiye bağlı astigmatizmada nötralizasyona ulaşmayı değerlendirmek Gereç ve Yöntem: Bu çalışma Kasım 1999 ve Mayıs 2001 tarihleri arasında Mini-Nük tekniği ile katarakt ameliyatı yapılan 66 hastanın 74 gözünü kapsamaktadır. Hastalar kesinin lokalizasyonuna göre iki gruba ayrıldı. Birinci grupta 5.5-6.5 mm'lik dış kesi sağ ve sol gözde 90 derece ekseninde, limbusun 1 mm gerisinde düz kesi şeklinde uygulandı. Bu grupta cerrahi öncesi astigmatizması 1D'den az olan 46 olgu mevcuttu. İkinci grupta 5.5-6.5 mm'lik dış kesi sağ gözde 135 derecede, sol gözde 45 derecede, limbusun 1 mm gerisinde ve düz kesi şeklinde uygulandı. Bu grupta ise cerrahi öncesi astigmatizması 1 D'den az olan 28 olgu mevcuttu. Bulgular: Hastalar en az altı ay süre ile takip edildiler. Birinci grupta cerrahiye bağlı olarak görülen absolü astigmatizma 2.27 D olarak gerçekleşti. İkinci grupta cerrahiye bağlı olarak görülen absolü astigmatizma ise 1.30 D olarak gerçekleşti. İkinci gruptaki olguların %95'inde absolü cerrahi astigmatizma birinci gruba göre belirgin bir şekilde daha az olarak gerçekleşti (t-testi: p<0.05). Sonuç: Mini-nük tekniği ile yapılan katarakt cerrahisinde, sklerkorneal tünel yapılanmasındaki 5.5-6.5 mm'lik düz kesi şeklindeki dış kesiyi üst-temporal tarafa doğru kaydırarak uygulamak cerrahiye bağlı absolü astigmatizmayı çok belirgin bir şekilde nötralize etmektedir.
Abstract
Purpose: To evaluate the results of varying the location of 5.5 to 6.5 mm external incision on the sclerocorneal tunnel formation in cataract surgery with Mini-nuc Technique for achieving postoperative astigmatic neutrality. Materials and Methods: This study comprised 74 eyes of 66 patients who received manual extracapsular cataract extraction (ECCE) with mini-nuc technique between November 1999 and May 2001. The follow-up was at least 6 months. The patients were divided into two groups according to external incision location. In the first group 5.5 to 6.5 mm straight shaped external incisions were placed at 90 degrees, 1 mm behind the limbus both of right and left eyes in which preexisting astigmatism of 1.00 diopter (D) or less. The number of cases were 46. In the second group the incisions were placed on the superior-temporal axis of each eye, 135 degrees(right eye) or 45 degrees(left eye) in which preexisting astigmatism was 1.00 D or less. The number of cases were 28. Results: In the first group surgically induced astigmatism (SIA) was 2.27 D. In the second group SIA was 1.30 D. Ninety-five percent of cases had less surgically induced astigmatism than the first group. Student- t test (p<0.05). Conclusion: In cataract surgery with Mini-nuc Technique, sclerocorneal self-sealing incisions using the BENT (between 9 and 12 o'clock) straight shaped external incision effectively achieved postoperative astigmatic neutrality.
