Dirençli gelişimsel glokom olgularında mitomisin C ile trabekülektomi

Özet
Amaç: Dirençli gelişimsel glokom olgularında mitomisin C (MMC) ile trabekülektomi ameliyatının etkinliği ve güvenirliliğinin araştırılması. Gereç ve Yöntem: MMC ile trabekülektomi ameliyatı uygulanan 6 primer konjenital glokom, 1 Axenfeld-Rieger anomalisi (2 göz), 1 aniridi (2 göz) hastasının 12 gözüne ait sonuçlar retrospektif olarak incelendi. Sonuçların istatistiksel değerlendirmesinde paired t test ve ki kare testi kullanıldı. İstatistiksel anlamlılık p<0.05 olarak kabul edildi. Bulgular: Hastaların 6'sı erkek, 2'si kızdı. Yaş ortalamaları 6.4±3.9 yıldı. On gözde başarısız trabekülotomi (2 kez) veya trabekülektomi hikayesi mevcuttu. Aniridi olgusunda MMC ile trabekülektomi, primer cerrahi olarak tercih edildi. MMC tüm olgularda 0.2 mg/ml 2 dak. süreyle uygulandı. Hastalar ameliyat sonrası ortalama 16.5±5.1 ay süreyle takip edildiler. Ameliyat öncesi göz içi basıncı (GİB) ortalaması 30.2±4.9 mmHg olan olguların, ameliyat sonrası GİB ortalaması 14.4±4.6 mmHg olarak saptandı. Ameliyat öncesi tüm olgular 2-3 adet (ort: 2.4±0.5) antiglokom ilaç kullanmaktayken, ameliyat sonrası yalnız 4 olguda ek ilaç gereksinimi oldu (ort:0.33±0.6). GİB düşüşü ve ilaç sayısındaki azalma istatistiksel olarak anlamlıydı (p<0.001). Son kontrol muayeneleri baz alındığında tüm olgularda ilaçlı veya ilaçsız GİB kontrol altında olup, değerlendirilebilen olgulardan hiçbirinde görme keskinliğinde azalma saptanmadı. Ameliyat sonrası erken dönem 4 olguda hipotoni (4-6 mmHg) sorunu ile karşılaşıldı. Ancak hepsinde ek girişime gerek kalmadan 7-10 gün içerisinde kendiliğinden düzelme saptandı. Sonuç: Dirençli gelişimsel glokom olgularında düşük doz ve uygulama süresi ile gerçekleştirdiğimiz MMC (0.2mg/ml 2 dak.) ile trabekülektomi ameliyatını, etkili ve güvenli bir cerrahi yöntem olarak değerlendirmekteyiz. Ancak bu konuda sınırlı sayıda tecrübemiz olup, farklı konsantrasyon ve uygulama süresi ile daha uzun takip dönemini kapsayan, geniş olgu serili çalışmalara ihtiyaç olduğunu düşünmekteyiz.
Abstract
Purpose: To evaluate the efficacy and safety of trabeculectomy with mitomycin-C (MMC) in refractory developmental glaucoma cases. Material and Methods: The outcomes of trabeculectomy with MMC were investigated retrospectively in 12 eyes of 8 patients; 6 patients with primary congenital glaucoma, 1 Axenfeld-Rieger abnormality (2 eyes) and 1 patient with aniridia (2 eyes). Statistical analysis were performed using "paired t" and "chi-square" tests at 0.05 significance level. Results: There were 6 male and 2 female patients in this study group. Average age of the study group was 6.4±3.9 years. There were histories of unsuccesful trabeculotomy (twice) or trabeculectomy in 10 eyes. Trabeculectomy with MMC was the primary choice of surgery for the patient with aniridia. In all cases MMC was applied to the sclera in 0.2 mg/ml concentration for 2 minutes. The mean follow-up period was 16.5±5.1 months. Intraocular pressures and antiglaucomatous medication usage decreased manifestly in the postoperative period. The mean preoperative IOP which was 30.2±4.9 mmHg, decreased to 14.4±4.6 mmHg, and mean preoperative antiglaucomatous medication usage which was 2.4±0.5, decreased to 0.33±0.6 in the postoperative period. The reductions in the IOP and number of antiglaucomatous medication were statistically significant (p<0.001). Loss in Snellen visual acuity lines was not detected in any of the patients during the follow-up period and IOPs were normal in all patients with or without medication. There were 4 cases of ocular hypotony which normalized spontaneously during the follow-up period. Conclusion: Trabeculectomy with MMC seemes to be an effective and safe method of surgery in refractory developmental glaucoma cases. However, it is very difficult to reach a strict conclusion about the method with this small study group. Therefore, further investigation is needed in large series of patients using different concentrations and application periods to evaluate effectivity and safety of the procedure.
