Özet
Bu çalışmada, katarakt cerrahisi uygulanacak olgularda cerrahi sonrası düzeltilmiş en iyi görme keskinliklerini tahmin etme konusunda "Heine Lambda 100 Handheld Retinometer" in güvenilir¬liği araştırıldı. 144 olgunun 145 gözünde preoperatif tahmin edilen görme keskinliği değerleri, postoperatif düzeltilmiş en iyi görme keskinliği değerleri ile karşılaştırıldı. SneHen eşelinde ± 2 sıra yanılma payı ile doğru tahmin oranı % 53.1, yanlış negatif (■ 3 sıra ve altı yanılma ) oranı % 46.2, yanlış pozitif (+ 3 sıra ve üzeri yanılma) oranı % 0.7 olarak saptandı. Preoperatif düzeltilmiş en iyi görme keskinliği 0.1 in altındaki olgularda (Grup I) doğru tahmin oranı % 29.5, yanlış nega¬tif oranı % 70.5, yanlış pozitif oranı % 0, preoperatif düzeltilmiş en iyi görme keskinliği 0.1 ve üs¬tündeki olgularda (Grup II) doğru tahmin oranı % 80.6, yanlış negatif oranı % 17.9, yanlış pozitif oranı % 1.5 olarak bulundu. İki grup arasındaki tahmin doğruluğu istatistiksel açıdan ileri dere¬cede farklıydı ( p= 0.000 ). Sonuç olarak Heine Retinometre'nin, özellikle nonmatür kataraktlı ol¬guların postoperatif görsel prognoz tayininde yararlı olabileceği kanısına varıldı.
Abstract
In this study we investigated the accuracy of predicting final visual acuity with "Heine Lambda 100 Handheld Retinometer" in cataract patients. Estimated visual acuities were compared to the postoperative best corrected visual a ities in 145 eyes of 144 cataract patients. On Snellen chart, overall success rate of prediction within two lines was 53.1 %. The rate of false negative results, that is 3 or more lines than predicted, was 46.2 %. The rate of false positive results, that is 3 or less lines than predicted, was only 0.7 %. In Group I, which consisted the patients with preoperative best corrected visual acuities worse than 0.1, the rate of accurate estimation was 29.5 %, the rate of false negative results was 70.5 %, and the rate of false positive results was 0 %. In Group II, those with preoperative best corrected visual acuities better than 0.1, accuracy of estimation was 80.6 %, the rates of false negative and positive results were 17.9 % and 1.5 %, respectively. The accuracy of estimation was statistically different between the two groups (' p=0.000). It can be concluded that "Heine Lambda 100 Handheld Retinometer" may be beneficial in assessment of postoperative visual prognosis, especially for the patients with nonmature cataract.
