Özet
Amaç: Bu çalışmada, kliniğimizde değerlendirilen diyabetik retinopatili olgulardaki asimetrik diyabetik retinopati (ADR) sıklığı ve asimetrik retinopati gelişimine etkili faktörler araştırıldı. Gereç ve Yöntem: Ocak-Aralık 2002 tarihleri arasında kliniğimizde fundus floresein anjiyografi ile değerlendirilen 100 diyabetik retinopatili olgu çalışma kapsamına alındı. ADR, bir gözde proliferatif DR mevcutken diğer gözde diyabetik retinopatinin olmaması veya zemin DR bulunması olarak değerlendirildi. Ayrıca, bir gözde diyabetik makülopati varken diğer gözde retinopatinin olmaması da ADR içinde kabul edildi. Bulgular: ADR 5 olguda tespit edildi (%5). ADR tespit edilen olguların 4'de proliferatif DR gelişen tarafta retinal ven tıkanıklığı (RVT) bulguları tespit edildi. ADR tanısı konan son olgunun sağ gözünde klinik açıdan önemli maküla ödemi saptanırken diğer gözde retinopati saptanmadı. Retinopatisiz gözde koruyucu faktör olarak ambliyopi tespit edildi. Sonuç: Sonuç olarak, asimetrik diyabetik retinopatili olgularda asimetrik etken olarak RVT ve ambliyopi tespit edildi. ADR ile karşılaşıldığında bu duruma yol açan lokal veya sistemik nedenler ayrıntılı olarak araştırılmalıdır.
Abstract
Purpose: The frequency of asymmetric diabetic retinopathy (ADR) and factors associated with this condition were investigated in diabetic patients evaluated in our clinic. Material and Methods: One hundred diabetic patients evaluated with fundus fluorescein angiography between January-December 2002 were included in this study. ADR was defined as proliferative DR in one eye and background or no diabetic retinopathy in the fellow eye. Additionally, presence of diabetic maculopathy in one eye and no retinopathy in the other eye was also evaluated in the context of ADR. Results: ADR was detected in 5 patients (5%). In four subjects, findings of retinal vein occlusion (RVO) were observed in eyes with proliferative DR. The last patient with ADR had clinically significant macular edema in his right eye and no retinopathy in the fellow eye. The protective factor in the left eye was determined to be amblyopia. Conclusion: RVO and amblypia were detected as asymmetric factor in our cases with ADR. Local and systemic factors which result in development of ADR should be investigated in detail in patients with this condition.
