Diyabetik retinopatili hastalarda kistoid maküla ödemi üzerine etkili risk faktörleri

Özet
Amaç: Diyabetik retinopatili hastalarda kistoid maküla ödemi (KMO) gelişiminde etkili olan risk faktörlerini belirlemek. Gereç ve Yöntem: Kliniğimize başvuran 788 diyabetik retinopatili hastanın 1576 gözü retrospektif olarak incelendi. KMO'sü olan 53 hastanın 66 gözü çalışma grubu olarak tanımlanırken KMO'sü olmayan diyabetik retinopatili 735 hastanın 1470 gözü kontrol grubu olarak alındı. KMO üzerine etkileri araştırılan faktörler yaş, cinsiyet, diyabetin tipi, diyabetin başlangıç yaşı, diyabetin süresi, diyabetin tedavi şekli, hemodiyaliz, hipertansiyon (HT), glikolize hemoglobin (HbAIc) düzeyi ve diyabetik retinopatinin (DR) evresi idi. KMO ve bu faktörler arasındaki çok değişkenli ilişki lojistik regresyon analizi ile değerlendirildi. Faktörlerin birbirinden bağımsız olarak KMO gelişimi üzerine etkilerinin incelenmesinde Ki-kare ve Student-t testleri kullanıldı. Bulgular: Lojistik regresyon analizinde, KMO gelişiminde istatistiksel olarak anlamlı etkileri olan faktörlerin HbAIc düzeyi ve diyabetin tipi olduğu bulunurken (p 0.05). Faktörler birbirinden bağımsız incelendiğinde ise, yüksek HbAIc düzeyi, Tip l diyabet veproliferatif DR'li olgularda KMO'nün istatistiksel olarak anlamlı düzeyde daha fazla olduğu görülürken (p 0.05). Sonuç: Yüksek HbAIc düzeyi, proliferatif diyabetik retinopati ve Tip l diyabetin KMO için risk faktörleri olarak bulunması, diyabetik hastalarda düzenli oftalmolojik muayene ve iyi bir metabolik kontrolünün önemini göstermektedir.
Abstract
Purpose: To determine the risk factors for cystoid macular edema (CME) in patients with diabetic retinopathy. Material and Method: Fifteen hundred and seventysix eyes of 788 patients attending to our clinic due to diabetic retinopathy -were retrospectively analyzed. Sixtysix eyes of 53 patients with CME were named as study group; whereas, 1470 eyes of 735 patients without CME were taken as control group. Age, gender, type of diabetes, age at the onset of diabetes, duration of diabetes, type of diabetes treatment, hemodialyses, hypertension (HT), hemoglobin AIc (HbAIc) level and degree of diabetic retinopathy (DR) were the factors which were evaluated for their effects on CME. The multivariate-adjusted association between these factors and CME was evaluated by logistic regression analysis. Chi-square and Student-t tests were used to investigate the independent effects of these factors on CME. Results: As analysed by multiple logistic regression model, HbAIc level and type of diabetes were found to be the statistically significant factors for CME (p<0.05); whereas, the effects of age, gender, age at the onset of diabetes, duration of diabetes, type of diabetes treatment, hemodialysis, HT and degree of DR on CME were found to be statistically insignificant (p>0.05). On the other hand, when the factors were investigated independently, high HbAIc level, Type I diabetes andproliferative DR were the statistically significant factors for CME (p<0.05); whereas, age, gender, age at the onset of diabetes, duration of diabetes,, type of diabetes treatment, hemodialysis and HT did not have a statistically significant effect on CME (p>0.05). Conclusion: High HbAIc level, proliferative DR and type I diabetes were found to be the risk factors for CME. This emphasizes the importance of regular ophthalmologic examinations and good metabolic control in diabetic patients.
