Ön segment gelişim anomalilerinde tanı dağılımı ve hastaya yaklaşım

Özet

Amaç: Kliniğimizde ön segment gelişim anomalisi saptanan olgularda tanı dağılımı ve tedavi yöntemlerinin incelenmesi. Gereç ve Yöntem: Son 10 yıl içinde Gazi Üniversitesi Tıp Fakültesi göz kliniğine başvuran ön segment disgenézili hastalara ait dosya kayıtları, geliş şikayetleri, bulguları, konulan tanılar ve yapılan cerrahi girişimler açısından retrospektif olarak incelendi. Bulgular: Çalışmaya 16 hastanın 32 gözü dahil edildi. 10'u kadın, 6'sı erkek olan olguların yaşları 1-50 ay arasında değişmekteydi. En sık başvuru nedeni diğer hastanelerden glokom cerrahisi (%43.7) veya parsiyel penetran keratoplasti~(%31.2) cerrahisi için sevkedilmeleri idi. %18.7'sinde gözde korneada-beyazlaşma şikayeti mevcuttu. Tanı dağılımına bakıldığında ise olguların %50'sinde Axenfeld Rieger, %25 'inde Peters ve kalan %25 'inde de mikrokornea ile birlikte lökom ya da anterior 'Sinesi olduğu görüldü. Bu olgular içinde 6 göze trabekülektomi, bir göze trabekülotomi, seton cerrahisi ve goniotomi ve 7 göze parsiyel penetran keratoplasti (PPK) uygulanmıştı. Beş göze kombine glokom ve PPK cerrahisi uygulanmış, 22 göz ise konservatif olarak izlenmişti. Sonuç: Ön segment disgenezilerinde sınıflandırmada olduğu gibi tam konmada zorlanılan vakalar izlenilmektedir. Bu vakalarda'genetik çalışmalarla disgenezilerin daha iyi aydınlatılması ve sınıflandırılması mümkün olacaktır. Glokomla yakın beraberlikleri nedeniyle erken tanınmaları ve erken tedavileri önemlidir.

Abstract

Purpose: To evaluate the distribution of clinical entities within the group of anterior segment dysgenesis and treatment methods in this patient group. Material and Method: Data related to patients with anterior segment dysgenesis that applied to Gazi University Medical Faculty Ophthalmology department was evaluated respectively from the patient files. They were evaluated with respect to the chief complaint at the application, the distribution of clinical entities and the type of surgical intervention. Results: Thirty two eyes of 16patients (10 female, 6 male, mean age of 12.6 months) were included in the study. The main reason for the application was the referral from other hospitals for glaucoma (43.7%) arid keratoplasty (31.2%) surgery. 18.7% of the patients reported whitening of the cornea as the chief complaint. 50% of the patients had the diagnosis of Axenfeld Rieger, while 25% of them were diagnosed as Peters' and the other 25% as microcornea. Eyes with glaucoma underwent either trabeculectomy .(6 eyes), trabeculotomy (1 eye), tube-shunt surgery (1 eye) or goniotomy (1 eye). Seven eyes were treated with partial penetrating keratoplasty surgery. Five eyes had combined partial penetrating keratoplasty and glaucoma surgery. Twenty-two eyes were followed conservatively. Conclusion: Patients with anterior segment dysgenesis are sometimes hard to diagnose and classify. Genetic studies will help us to better evaluate and classify these anomalies. Follow up of these patients for early diagnosis and treatment of glaucoma is important due to their close relation with glaucoma.
