Addüksiyonda elevasyon (Upshoot) veya depresyon (Downshoot) gösteren Duane retraksiyon sendromlu olgularda cerrahi tedavi sonuçları

Özet
Amaç: Duane retraksiyon sendromlu (DRS) bazı olgularda addüksiyon esnasında elevasyon (upshoot) ya da depresyon (downshoot) gözlenmektedir. Ciddi kozmetik sorun oluşturan böylesi durumlarda cerrahi tedaviye başvurmak gerekebilir. Amacımız bu tür hastalarda uyguladığımız cerrahi teknik ve sonuçları sunmaktır. Gereç ve Yöntem: Bu çalışma, yaşları 21-23 arasında değişen ve addüksiyonda belirgin elevasyon veya depresyon gösteren altı erkek olgu üzerinde gerçekleştirildi. Elevasyon veya depresyonu düzeltmek için, lateral rektus kasa Y-split cerrahisi uygulandı. Y-split işleminin yeterli olmadığı durumlarda üst rektus kas cerrahisi (geriletme ve/veya Faden) ve/veya alt oblik kas miyektomi yapıldı. Bulgular: Olguların dördü unilateral iken, iki olgu bilateraldi. Yine, olgulardan dördü tip l iken, ikisi tip 2 idi. Olgulardan beşinde elevasyon varken, bir olguda ise depresyon vardı. Ayrıca, primer pozisyonda 4 olguda ekzotropya vardı. Lateral rektus Y-split cerrahisi beş olguda tek başına veya üst rektus/alt oblik kas cerrahisi ile kombine edilerek uygulandı. Bu cerrahi girişimler sonrası tüm olgularda elevasyon veya depresyonun tatminkar bir şekilde düzeldiği gözlendi. Sonuç: Addüksiyonda gözün yukarı veya aşağı deviasyonu DRS'lu olguların kliniğinde önemli bir yer tutmaktadır. Bir çok hastada bu durum bir sorun oluşturmazken, bazı olgularda ise ciddi kozmetik sorunlara neden olabilir. Böylesi durumlarda cerrahi tedaviye başvurulabilir ve tatminkar sonuçlar elde edilebilir.
Abstract
Purpose: In some cases with Duane's retraction syndrome elevation or depression can be seen in adduction. A surgical procedure can be necessary in cases that have serious cosmetic problems. Our aim is to declare our surgical technique and the results of the operation in these patients. Material and Methods: Procedures were applied in six male patients that had distinct elevation or depression in adduction. They were 21 to 23 years of age. Y-split surgical procedure was applied to lateral rectus muscle for correcting the elevation or depression. Superior rectus muscle operation (recession and/or Faden operation) and/or inferior oblic muscle myectomy were applied in cases which Y-split procedure was not enough. Results: In two of the six patients both eyes had been involved and in the other patients only one eye had been involved. Two of the cases were type II and four of the cases were type I. One of the patients had depression and the other five patients had elevation. At the same time, exotropia were present in four cases in primary position. Lateral rectus muscle Y-split surgical procedure was applied solely or combined with superior rectus/inferior oblic muscle operation. Elevation and depression disappeared reasonably in all patients. Conclusions: Elevation or depression in adduction with Duane's retraction syndrome is a particular clinical form. This occasion may cause serious cosmetic problems in some patients, but in most of the patients do not. Surgical procedure solves this cosmetic problem successfully in these cases.
