Konjenital glokom olgularında cerrahi sonuçlarımız

Özet
Amaç: Konjenital glokom olgularında uyguladığımız cerrahi yöntemlerin (trabekülotomi, trabekülektomi, kombine cerrahi) sonuçlarını sunmayı ve cerrahi seçimde dikkat edilmesi gereken noktaları vurgulamayı amaçladık. Gereç ve Yöntem: Konjenital glokom tanısı ile ameliyat edilen 19 hastanın 32 gözüne ait kayıtlar geriye dönük olarak değerlendirildi. Bulgular: Hastaların 14'ü erkek (%74), 5'i kız olup, 13 olgu iki taraflı (%68), 6 olgu tek taraflıydı. Cerrahi uygulama yaşı ort: 38.4±27 ay idi. Yirmibeş gözde saf trabeküler disgenezis (%78), 7 gözde iridotrabeküler disgenezis mevcuttu. Oniki hastada akraba evliliği hikayesi vardı (%63) ve %58 hasta Güneydoğu Anadolu bölgesinden kliniğimize sevkedilmişti. On göze trabekülotomi, 10 göze trabekülektomi, 12 göze kombine cerrahi uygulandı. Genel olarak ameliyat öncesi göz içi basıncı(GİB) ort: 31.7±3.9 mmHg, c/d oranı 0.65±0.2 ve horizontal kornea çapı 13.9±0.70 mm olarak tesbit edildi. Olgular ameliyat sonrası ort: 24.1±11.9 ay süreyle takip edildiler. Son kontrol muayene bulguları baz alındığında GİB ort: 16.9±3.6 mmHg, c/d oranı 0.60±0.2 ve kornea çapı 13.8±0.75 mm olarak saptandı. Ameliyat öncesi ve sonrası bulgular istatistiksel olarak karşılaştırıldığında GİB düşüşü ve c/d oranı değişim değerleri arasında anlamlı fark olduğu belirlendi (p 0.05). Takip süresi sonunda trabekülotomi grubunda başarı oranı %90, trabekülektomi grubunda %70, kombine cerrahide %100 olarak gerçekleşti. Sonuç: En yüksek başarı oranımız kombine cerrahi grubunda olup, gözdeki anatomik yapı anomalileri ve yaş faktörü dikkate alınarak yapılan cerrahi seçimin başarı oranını artırabileceği kanısındayız.
Abstract
Purpose: To present the outcome of surgery performed in congenital glaucoma cases (trabeculotomy, trabeculectomy, combined surgery) and to emphasize the factors to consider when deciding on the type of surgical procedure. Materials and Methods: The records of 32 eyes of 19 patients with congenital glaucoma who had undergone operation were retrospectively evaluated. Results: There were 14 (74%) male and 5 female patients. The mean age at surgical performance was 38.4±27 months. Twenty five eyes had pure trabecular dysgenesis (78%) and 7 eyes iridotrabecular dysgenesis. Twelve cases were offsprings consanginous marriages (63%). Fifty eight percent of the patients were reffered to our clinic from southeastern Anatolia. Trabeculotomy was performed in 10 eyes, trabeculectomy in 10 eyes and combined surgery in 12 eyes. Mean preoperative intraocular pressure(IOP) was 31.7±3.9 mmHg, mean cup/disc ratio was 0.65±0.2 and mean horizontal corneal diameter was 13.9±0.70 mm. Patients were followed for a mean of 24.1±11.9 months. At last follow up visit mean IOP was 16.9±3.6 mmHg, cup/disc ratio was 0.60±0.2 and corneal diameter was 13.8±0.75 mm. The difference between mean preoperative and postoperative IOP and cup/disc ratios were statistically significant (p<0.001). The difference between preoperative and postoperative horizontal corneal diameters was not statistically significant (p>0.05). At last follow up, the success rate in the trabeculotomy group was 90%, trabeculectomy group was 70% and combined surgery group was 100%. Conclusion: Combined surgery was found to be the most successfull. It is thought that, surgical decision made by taking into account the age and anatomic structural anomalies would increase the success rate.
