Odunsu konjonktivitli bir olguda oral tutulumla birlikte düşük plazminojen aktivitesi

A ligneous conjunctivitis case with low plasminogen activity and gingival involvement

Medikal tedaviye dirençli membranöz konjonktivit tablosuyla başvuran, 12 aylık erkek hastadan konjonktiva kültürleri alındı. Kültürlerde üreme olmadı. Hastaya membran eksizyonu yapıldı ve materyel patolojik incelenmeye yollandı. Biyopsi sonucunda, fibrinoid reaksiyon ve hiyalinize membran saptandı. Odunsu konjonktivit tanısı kondu. Topikal steroid damla ve merhem, topikal antibiyotik, topikal antiallerjik ve Siklosporin-A %2'lik damla başlandı. Membranlar birkaç günde tekrarladı. Plazminojen aktivitesi %58 (normal %80-%100) olarak tespit edildi. Hastada bu tedaviyle semptomatik rahatlama sağlandı. Bir ay sonraki kontrolde gingivada sarı-beyaz renkli yuvarlak membranlar tespit edildi. Tedaviye dirençli membranöz konjonktivitlerde odunsu konjonktivit ayırıcı tanıda hatırlanmalı ve plazminojen eksikliği saptanırsa birlikte sistemik tutulumun da olabileceği akılda tutulmalıdır.

Abstract

A 12 months old boy presented with a history of membranous conjonctivitis. Conjunctivitis was resistant to medical therapy. Conjunctival cultures were taken. No growth detected in cultures. Membranes were excised, and sent to pathological examination. Biopsy results revealed fibrinoid reaction and hyalinized membrane. The diagnosis was ligneous conjunctivitis. Topical steroid drops and ointment, topical antibiotics, topical antiallergic and topical cyclosporin-A 2% were given as medical therapy. Membranes recurred in a couple of days. Plasminogen activity was detected as 58% (normal 80%-100%). With this therapy we observed symptomatic relief in the patient. In the 1 month control, we detected yellow-white, round shaped membranes on the gigiva. In membranous conjuntivitis cases which are resistant to medical therapy, ligneous conjunctivitis should be included in the differantial diagnosis, and if low plasminogen activity is detected, and systemic involvement should be examined.
