Özet

Amaç: Disosiye vertikal deviasyon (DVD) ile birlikte gözlenen kaymaların tespiti ve uygulanan cerrahi metotların etkinliğini değerlendirmek. Gereç ve Yöntem: DVD'H 9 olguda uygulanan horizontal cerrahiye ek olarak, üst rektus kasına geriletme, alt oblik kasın öne transpozisyonu, alt oblik kasına geriletme veya myektomi, üst oblik kasa posterior tenektomi ve alt rektus kasın rezeksiyon sonuçları retrospektif olarak incelendi. Bulgular: Yaşları 6-28 arasında değişen olguların yaş ortalaması 14.3 yıl idi. DVD başlama yaşı ortalama 3.2 yaş, %89 (8 olgu) bilateral, %67 (6 olgu) asimetrik olarak tespit edildi. Olguların %56'sında (5 olgu) A patern, %44'ünde (4 olgu) V patent izlendi. Altı olguda (%67) bir seansta, 8 olguda (%89) 2 sensta, tümünde (%100) ise 3.seansta uzakta primer bakışta örf oryaya ulaşılırken, olguların hiçbirinde peroperatuvar veya postoperatuvar komplikasyona rastlanmadı. Sonuç: Kozmetik sorunlar yaratan DVD'nin tedavisi, doğru tanı ve uygun cerrahi yaklaşımlar ile mümkün olmaktadır.

Abstract

Purpose: To determine the associated deviations to dissociated vertical deviation (DVD) and to evaluate the effectiveness of surgical approaches. Material and Method: This study was a retrospective analysis of the surgical treatments like recession of the superior rectus muscle, anterior transposition, myektomi or recession of the inferior oblique muscle, posterior tenectomy of the superior oblique muscle and resection of the inferior rectus muscle to addition to the horizontal rectus surgery in 9 patients with DVD, which were diagnosed and followed up. Results: The ages of the patients ranged between 6 and 28 years (mean, 14.3 years). Beginning age of the DVD was 3.2 years. In eight patients (89%), DVD was bilateral, in six patients (67%) asymmetrical. 56% of patients (5 patients) had A pattern, 44% of patients _ (4 patients) had V pattern. In 6 patients (67%) in one, in 8 patients (89%) in two and all patients (100%) in three surgery, planned recovery of DVD was attained. No peroperative and postoperative complications were encountered. Conclusion: The treatment of DVD that gives cosmetically trouble, is possible with true diagnosis md appropriate surgical approaches.
