Hiperhomosisteineminin eşlik ettiği bir santral retinal arter oklüzyonu: Olgu sunumu

Özet
Santral retinal arter oklüzyonu geçiren ve artmış plazma homosistein seviyesi tespit edilen bir olguyu sunmak. Ellialtı yaşında erkek hasta sol gözünde ağrısız ani görme kaybı şikayeti ile kliniğimize başvurdu. Oftalmoskopide; retinanın soluk ve ödemli olduğu, makulada Japon bayrağı görüntüsü ve arterlerde atenüasyon izlendi. Eritrosit sedimentasyon hızı, hematolojik faktörler, plazma homosistein düzeyi ve homosistein düzeyini etkileyebilecek; vitamin B12, folik asit ve kreatinin düzeyleri çalışıldı. Oral glukoz tolerans testinde bozukluk ve hiperlipidemi haricinde başka bir sistemik patoloji tespit edilmedi. Emboli kaynağı olabilecek karotis arter ve kardiyak patoloji saptanmadı. Pıhtılaşma faktörleri (PT, PTT, protein C ve protein S) normal sınırlarda idi. Plazma homosistein seviyesi yüksek bulundu (27.4 µmol/L; normal: 5-15 µmol/L). Vitamin B12 düzeyi düşük bulundu ve parenteral vitamin B12 tedavisi başlandı. Bir ay sonraki kontrol homosistein düzeyi 8.6 µmol/L olarak bulundu. Artmış plazma homosistein seviyeleri retinal arter oklüzyonu için bağımsız bir risk faktörü olabilir ve bu risk faktörü vitamin B12 ve folik asit tedavisi ile normal seviyelere indirilebilir.
Abstract
To present a case with central retinal artery occlusion (CRAO) associated with increased plasma homocysteine. A fifty-six-year old male patient applied to our clinic with the complaint of painless loss of vision in his left eye. The ophthalmoscopy revealed pale, edematous retina and cherry-red spot at macula with arterial attenuation. In addition to erythrocyte sedimentation rate, hematological factors, blood level of homocysteine and factors which may affect plasma homocysteine (vitamin B12, folic acid, creatinine) were measured in plasma. Other than impaired glucose tolerance test and hyperlipidemia, no systemic pathology could be identified. No embolic source from heart or carotid arteries were found. Hematological factors (PT, PTT, protein C and S) were within normal limits. Plasma homocysteine level was found to be elevated (27.4 µmol/L; normal: 5-15 µmol/L). Vitamin B12 was measured to be low and parenteral treatment was instituted. Control homocysteine level obtained one month later was 8.6 µmol/L. Elevated plasma levels of homocysteine might be an independent risk factor for CRAO and this risk factor may be treated with vitamins.
