Fakoemülsifikasyon katarakt cerrahisinde kullanılan viskoelastiklerin postoperatif göziçi basıncı üzerine etkileri

Özet

Amaç: Fakoemülsifikasyon katarakt cerrahisinde kullanılan Viscoat (kondroitin sülfat %4+ sodyum hyalüronat %3, Alcon)+Healon (sodyum hyalüronat %1, Pharmacia) ve Viscoat+Biolon (sodyum hyalüronat %1, Chauvin) kullanımının ameliyat sonrası göziçi basıncı (GİB) üzerine etkilerinin karşılaştırılması. Gereç ve Yöntem: Tek cerrah tarafından fakoemülsifikasyon ve kapsüliçi göziçi lens implantasyonu uygulanan 55 hasta çalışma kapsamına alındı. Hastalar randomize olarak iki gruba ayrıldı ve ameliyat sırasında olarak birinci grupta 35 göze Viscoat+Healon, ikinci grupta 20 göze Viscoat+Biolon kullanıldı. Postoperatif L, 3., 7., 15. günlerde göziçi basınçları kaydedildi. Bulgular: Ameliyat öncesi GİB 'lan 1. grupta ortalama 13.3±2.8 mmHg, 2. grupta 13.0±1.5 mmHg olarak bulundu (p = 0.764). Ameliyat sonrası 1. günde ortalama göziçi basıncı 1. grupta 14.8±5.9 mmHg, 2. grupta 14.l±5.2 mmHg (p=0.712), 3. günde ortalama göziçi basıncı 1. grupta 11.6±1.8 mmHg, 2. grupta 10.7±1.2 mmHg (p=0.127), 7. günde ortalama göziçi basıncı 1. grupta ll.3±1.6 mmHg, 2. grupta 10.7±0.8 mmHg (p=0.273), 15. günde ortalama göziçi basıncı 1. grupta 11.2±1.3 mmHg, 2. grupta 10.5±1.3 mmHg-olarak saptandı (p=0.136). L, 3., 7. ve 15. günlerde ameliyat sonrası göziçi basıncı değerleri açısından Viscoat+Healon (Grup 1) ve Viscoat+Biolon (Grup 2) arasında istatistiksel anlamlı fark saptanmadı. Sonuç: Değişik Viskoelastiklerin ön-arka kamara ve kapsüler kese içinden yeterli ve etkin düzeyde irrigasyon ve aspirasyon yoluyla temizlenmesi halinde fakoemülsifikasyon sonrası göziçi basıncı etkilerinde bir fark gözlenmemektedir. Bu nedenle dikkatli ve yeterli irrigasyon-aspirasyon yapılırsa cerrahide kullanılabilecek viskoelastik madde seçenekleri artmaktadır.

Abstract

Purpose: To compare the effects of Viscoat (chondroitin sulfate 4%+ sodium hyaluronate 3%, Alcon)+Healon (sodium hyaluronate 1%, Pharmacia) and Viscoat+Biolon (sodium hyaluronate 1%, Chauvin) on postoperative intraocular pressure (IOP) after phacoemulsification cataract surgery. Material and Method: Fifty-five patients who underwent phacoemulsification and in-the bag intraocular lens implantation by a single surgeon constituted the study group. The subjects were randomly assigned into two groups; the first group (35 eyes) received Viscoat+Healon, the second group received Viscoat+Biolon intraoperatively. Intraocular pressures were recorded at postoperative 1,3,7 and 15 days, Results: Preoperative lOPs were 13.3±2.8 and 13.0±1.5 mmHg for the first and second group, respectively (p=0.764). lOPs at postoperative days were as follows; at day 1; 14.8±5.9 mmHg and 14.1±5.2 mmHg (p=0.712), at day 3; 11.6±1.8 mmHg and 10.7H.2 mmHg (p=0.127), at day 7; 11.3±1.6 mmHg and 10.7±0.8 mmHg (p=0.273), at day 15; 11.2±.1.3 mmHg and 10.5±1.3 mmHg (p=0.136) for the Group 1 and 2, respectively. No statistically significant difference was observed between the two groups in terms of postoperative IOP. Conclusion: No difference was observed regarding the effect of different viscoelastic substances on the postoperative IOP following phacoemulsification when they were effectively removed from the anterior-posterior chamber and capsular bag with irrigation and aspiration. Hence, a careful and sufficient irrigation and aspiration may increase the alternatives among viscoelastics.
