Özet

Amaç: Sağ gözü konjenital glokom, sol gözü juvenil glokom tanısı almış bir olgunun, aile bireylerinin taranması sonucunda elde edilen 6 kuşak jenerasyona ait pedigrinin klinik ve genetik özellikleri incelendi. Gereç ve Yöntem: Otuzaltı bireyden oluşan ve otozomal dominant geçiş özelliği gösteren pedigride, son 3 kuşak jenerasyona ait 18 kişi muayene edildi. Tüm olgularda ön segment muayenesi, göz içi basıncı (GİB) ölçümü, gonyoskopik muayene, arka segment ve optik disk muayenesi ile Humphrey görme alanı cihazıyla görme alanı muayenesi (30-2 eşik testi) yapıldı. Ulaşılabilen bireylerden CYP1B1 ve TIGR genlerine ait mutasyon taramaları için kan örnekleri alındı. Bulgular: Anamnez verileri ve oftalmoskopik muayene sonuçlarına göre pedigri bireylerinden 13 kişinin etkilendiği saptandı. Bu bireylerden indeks olgumuzun bir gözü dışında, 4. jenerasyondan bir olgu daha konjenital glokom olup, diğerleri juvenil glokomdu. Juvenil glokom olgularından 5'ine trabekülektomi ameliyatı uygulanmıştı. Bunlardan mitomisin C (MMC) kullanılan 4 olguda GİB kontrol altındaydı (12.2±1.5 mmHg). Bir olguda ise maksimal glokom tedavisiyle GİB her iki gözde de 24 mmHg düzeyindeydi. Taramalar sırasında indeks olgumuzun kardeşlerinden birinde daha juvenil glokom saptandı. İndeks olgumuzun sol gözüne trabekülotomi+trabekülektomi; GİB kontrol altında olmayan olguya, MMC ile iki taraflı tekrar trabekülektomi yapıldı. Yeni saptanan olguda ise, dorzolamid %2+timolol maleat %0.5 kombinasyonu ile GİB kontrolü sağlanabildi. Sonuç: Bu ailenin pedigrisinde olduğu gibi, juvenil glokom otozomal dominant geçiş göstermektedir. Bu nedenle hastaların akraba ve aile bireylerinin mutlaka gözden geçirilmesi ve olası glokom için gelecek kuşakların uyarılması gerekir.

Abstract

Purpose: The clinical and genetical features of a large pedigree consisting of 6 generations, which was discovered from a patient whom had congenital glaucoma in his right eye and juvenile glaucoma in the other eye, were evaluated in this study. Material and Method: Eighteen persons belonging to last 3 generations were examined in this otosomal dominant patterned pedigree, which was consisted of 36 persons totally. Anterior segment evaluation, gonioscopy, intraocular pressure measurement, fundoscopy and optic disc examinations, visual field examinations with Humphrey static perimetry were performed in each patient. Blood samples were obtained from the available individuals of the pedigree for the determination of possible mutations on the CYP1B1 and TIGR/MYOC genes. Results: Thirteen persons of the pedigree were proved to be affected according to the anamnestic findings and clinical examinations. There was only one case that had the diagnosis of congenital glaucoma other than one eye of our index case and the others were cases of juvenile glaucoma. Trabeculectomy operations had been performed on 5 eyes of juvenile glaucoma. Intraocular pressures (IOP) were normalized in 4 patients whom had trabeculectomy+mitomycine-C (MMC) operation, but IOPs were at 24 mm Hg despite maximum antiglaucomatous therapy in the other patient. Trabeculotomy+ trabeculectomy operation was performed in the left eye of our index patient and trabeculectomy+MMC operations were performed in both eyes of the patient whom we failed to control IOP with maximal antiglaucoma therapy. IOPs were normalized in both patients without medical therapy. IOP normalization was achieved in the new case by using dorzolamide %2+timolol maleate %0.5 combination. Conclusion: Juvenile glaucoma has an otosomal dominant inheritance pattern as seen in this pedigree. All of the family member must be regularly screened in order to avoid misdiagnosis and warn the next generation for probable glaucoma.
