Özet

Amaç: Kuru göz hastalarında punktum tıkaçlarının etkinliklerinin değerlendirilmesi ve silikon tıkaçların komplikasyonlarının tartışılması. Gereç ve Yöntem: Otuz iki hastanın 62 gözüne silikon tıkaç yerleştirildi. Hastaların 12'si (%37) erkek, 20'si (%62.5) bayandı. Olguların yaşları 30-75 yıl (ortalama 47±14.2) idi. Punktum tıkaçları yerleştirilmeden önce her iki göze topikal anestezi uygulanımından sonra Schirmer 1 testi yapıldı. Başlangıçta 32 hastada geçici kollajen tıkaçlar üst ve alt punktumlara yerleştirildi ve bir hafta içinde objektif ve subjektif olarak değerlendirildi. Uygun hastada öncelikle alt punktumlara kalıcı silikon tıkaçlar yerleştirildi. Takiplerde 62 gözün 12'sinde (%19.2) üst punktumlara kalıcı tıkaç gereksinimi oldu. Bulgular: Yirmisekiz (%87.5) hastada objektif ve subjektif düzelme oldu. Otuz (%93.8) hastada Schirmer 1 testinde artma oldu. İki hastada her iki gözde tedaviden sonra Schirmer testi 0 mm idi. Punktum oklüzyonundan 14 ay sonra bir hastanın bir gözünde piyojenik granülom gelişti. Bir hastanın bir gözünde (%3.1) arada sırada olan yaşarma, kaşıntı, ve yanma vardı. Ellidört gözde (%87.1) suni gözyaşı gereksinimi azaldı, 8 (%12.9) gözde ise değişmedi. Sonuç: Silikon punktum tıkaçlar, topikal lubrikan tedavi ile tek başına yanıt alınamayan orta ve ciddi kuru göz olgularında etkili olabilir. Silikon inert bir materyaldir ve genelde iyi tolere edilir, fakat nadiren komplikasyonlara neden olabilir. Bu nedenle bu hastaların takibi önemlidir.

Abstract

Purpose: To evaluate the efficiency and tolerability of plug treatment in dry eye patients, and to discuss the complications associated with punctal occlusion using silicon plugs. Material and Methods: Silicon plugs were used to treat 62 eyes in 32 patients. Twelve (37.5%) of the patients were male and 20 (62.5%) were female. The mean age in the group was 47±14.2 years (range, 30-75 years). For each eye, prior to punctal plug insertion, Schirmer 1 testing was performed with topical anesthesia. Initially, collagen plugs were inserted in all 32 patients to temporarily occlude the punctae. If there was no epiphora and subjective complaints, these plugs were removed and permanent silicone plugs were inserted in the lower punctae. Twelve (19.3%) of the 62 eyes required plug insertion in the superior puctae during follow-up. Results: Objective and subjective improvement was noted in 28 (87.5%) of the patients. Improvement in Schirmer 1 test results was achieved in 30 (93.8%) patients. Two (6.3%) patients had Schirmer 1 test results of 0 mm in both eyes after treatment. At 14 months after punctal occlusion, one eye in one patient developed pyogenic granuloma. Another patient (3.1%) developed occasional tearing, itching, and burning in one eye. In 54 (87.1%) eyes, the artificial tear requirements decreased, and in 8 (12.9%) eyes the requirements remained unchanged. Conclusion: Silicone punctal plugs can be effective in managing moderate to severe dry eye that is unresponsive to topical lubricant therapy alone. Silicone is an inert material that is generally well tolerated, but these plugs do occasionally cause complications. For this reason, follow-up is important in these patients
