Bakteriyel keratit tedavisinde steroid kullanımı

Özet

Gereç ve Yöntem: 1994-2004 tarihleri arasında kliniğe başvuran 106 bakteriyal keratitli olgu ret­ros­pektif olarak incelendi. Kornea ülseri, 58 olguda (%54.7) merkezi yerleşimliydi. Yetmiş sekiz (%73.6) ol­guda ülser çapı 2 mm’den büyüktü ve 66 (%62.3) olguda kornea ülserinin derinliği, kornea ka­lın­lı­ğı­nın %30’undan fazlaydı. Kültür örneği alınabilen 26 hastanın 18’inde (%69.2) kültürde üreme sap­tandı. Yi­rmi dokuz hastaya (%27.4) topikal antibiyotik tedavisine ek olarak topikal steroid (Grup 1A), 20 has­ta­ya ise (%18.9) oral steroid tedavisi uygulandı (Grup 1B). Elli yedi hastaya (% 53.8) ise to­pi­kal an­ti­bi­yo­tik tedavisine ilave olarak oral veya topikal steroid tedavisi uygulanmadı (Grup-2). Or­ta­la­ma 2.3±0.3 ay­lık takip sonunda antibakteriyal tedaviye ilaveten steroid kullanımının; iyileşme süresi ve görme kes­kin­liği üzerine etkisi araştırıldı. İstatistiksel değerlendirmelerde t-testi, paired t-test ve Wil­co­xon signed rank test ve Mann-Whitney U testi kullanıldı. İstatistiksel anlamlılık p 0.05). Grup 1A ve Grup 1B’de görme keskinliğindeki artış, Grup 2’deki görme keskinliğindeki artışa gö­re istatistiksel olarak anlamlı yüksek bulundu (p 0.05). Sonuç: Antibiyotik tedavisi ile birlikte kortikosteroid kullanımı, görme keskinliğinde artış sağ­la­mak­ta ve iyileşme süresinde uzamaya neden olmamaktadır.Topikal veya oral steroid kullanımı arasında gör­me keskinliğindeki artış açısından anlamlı bir fark yoktur.

Abstract

Purpose: To evaluate the effect of corticosteroids in combination with antimicrobial drugs in the treat­ment of bacterial keratitis, duration of treatment and final visual acuity. Matherial and Methods: The medical reports of 106 patients with a diagnosis of bacterial ke­ra­ti­tis were reviewed retrospectively between 1994 and 2004. Corneal ulcers were localized centrally in 58 patients (54.7%). Ulcer sizes were larger than 2 mm in 78 patients (73.6%) and the depth of the ul­cers were more than 30% of corneal thickness in 66 patients (62.3%). Eighteen cultures were positive in 26 patients. Corticosteroids were used in combination with antimicrobial drugs in 29 patients topically (27.4%) (Group 1A) and oral in 20 patients (18.9%) (Group 1B). Fifty eight patients (58.3%) were trea­ted only with antimicrobial drugs (Group 2). The mean follow-up time was 2.3±0.2 months. Results we­re compared using t-test, paired t-test , Wilcoxon signed rank test and Mann-Whitney U test. P value less than 0.05 was considered significant (p>0.05). Results: Duration of the treatment was 23±2 days in Group 1A, 22±2 in Group 1B and 21±2 days in Group 2. Visual improvement was equal or more than 2 lines in 96.6% of patients in Group 1A , 100% of patients in Group 1B and 58.9% of patients in Group 2. The difference between duration of treat­ment in groups were not statistically significant (p>0.05). Visual improvement between Group 1A and Group 1B is not statistically significant (p>0.05), but it was statistically significant between group 1 and group 2 (p<0.001). Conclusion: The use of coticosteroids in combination with antimicrobial drugs in bacterial ke­ra­ti­tis improve visual acuity but do not effect duration of treatment. There is no statistical significance in vi­sual improvement by using topical or oral steroids.
