Özet
Centurion sendromu, nadir olarak görülen ve çocuklarda ya da genç erişkinlerde fonksiyonel epiforaya yol açan bir medial kantal tendon anomalisidir. Epifora yakınmasıyla başvuran 31 yaşında erkek hasta muayene edildi. Her iki gözde alt göz kapaklarının kenarları medialde ve lateralde normale göre önde yerleşmişti ve globa değmiyordu. Biyomikroskopide alt lakrimal punktumlar alt göz kapağı çevrilmeksizin görülebiliyordu. Lakrimal irrigasyonda ve makrodakriyosistografide lakrimal drenaj yolunda tıkanma saptanmadı. Lakrimal sintigrafide sağda radyoaktif madde geçişi yoktu, solda gecikme belirlendi. Hasta, cerrahi olarak, lokal anestezi altında medial kantal tendonun ön demetinin daha posteriora tespit edilmesiyle tedavi edildi. Ameliyattan sonra 30 ay süreyle izlenen hastanın epifora yakınması belirgin biçimde azaldı. Centurion sendromu genç hastalarda fonksiyonel epiforanın ayırıcı tanısında göz önünde tutulmalıdır.
Abstract
Centurion syndrome is a rare anomaly of the medial canthal tendon causing functional epiphora in children or young adults. A 31 year-old male patient presenting with complaint of epiphora was examined. In both eyes the margins of the lower eyelids were located anteriorly compared to normal and did not touch the globe. In biomicrosopic examination, the lower lacrimal puncta could be seen without everting the lower eyelid. We did not find any obstruction in the lacrimal drainage pathway on lacrimal irrigation and macrodacryocystogram. On lacrimal scintigraphic examination the radioactive material did not pass through system in the right side, delaying of the passage was observed on the left side. The patient was treated surgically under local anesthesia by replacing the anterior crus of the tendon to a more posterior position. After the operation, the complaints of epiphora of the patient who was followed up 30 months was improved significantly. Centurion syndrome should be included in the differential diagnosis of functional epiphora in young patients.
