Penetran keratoplasti uygulanan keratokonüs olgularında greft ve alıcı yatak çapı farkının postoperatif refraksiyona etkisi

AMAÇ: Keratokonüs tanısı ile penetran keratoplasti uygulanan gözlerde sferik miyopinin greft çapının alıcı yatak çapı ile eşit tutulduğunda ne oranda azalacağı ve aksiyel miyopinin bunda etkili olup olmadığını araştırmak. GEREÇ VE YÖNTEM: Keratokonüs tanısı ile penetran keratoplasti uygulanan, greft çapının alıcı yatak çapından 0.25 mm büyük tutulduğu 20 göz ile aynı çapta tutulduğu 23 gözün, postoperatif ortalama 11. ayda korneal astigmatizma, objektif refraksiyon ve aksiyel uzunluk değerlerinin ortalamalarını tesbit ettik ve bu gözleri aksiyel uzunluk yönünden emetrop olan 25 kontrol gözü ile karşılaştırdık. BULGULAR: Greft ve alıcı yatak çaplarının aynı olduğu grupta ortalama sferik miyopi değerinin greft çapının büyük tutulduğu gruba göre istatistiksel olarak anlamlı derecede düşük olduğu ve keratokonüslü gözlerde aksiyel uzunluk ortalamasının kontrol grubuna göre daha yüksek olduğu görüldü. SONUÇ: Penetran keratoplasti uygulanan keratokonüslü gözlerde postoperatif sferik miyopiyi azaltmak için greft çapı ile alıcı yatak çapını eşit tutulmasının yararlı olabileceği düşünüldü.
Abstract
PURPOSE: To evaluate the effect of the difference between the sizes of donor button and host incision on postoperative spherical myopia at eyes with keratoconus to whom penetrating keratoplasty was performed. MATERIALS AND METHODS: Mean axial length, corneal astigmatism and objective refraction values of two penetrating keratoplasty group of patients at which 20 eyes with 0,25 mm oversized grafts and 23 eyes which the sizes of the donor button and the recipient were equal were compared. Mean axial length of the 25 control group patients was compared with the group of patients at whom the sizes of the donor button and recipient sizes were equal. RESULTS: At the end of the mean follow up period of 11 months, the mean spherical myopia value was found to be lower at the group with the same sized graft and when compared with the control group, the mean axial length value was higher at keratoconus patients. CONCLUSION: To hold the sizes of the donor button and the host incision equal seems suitable at penetrating keratoplasty for keratoconus.
