Özet
AMAÇ: Vizüel matürasyondan önce başlamış, erişkin yaşlarda tedavi için başvurmuş, primer veya nüks komitan erişkin şaşılıklı bir grup hastamız, tipleri, binoküler görmeleri ve yapmış olduğumuz şaşılık operasyonlarının sonuçlarını irdelemek için retrospektif olarak incelenmiştir. GEREÇ VE YÖNTEM: Yaşları 16-47 arasında (ortalama±standart sapma; 23±3.1) olan 51 komitan şaşılıklı hasta çalışma kapsamına alınarak retrospektif olarak değerlendirildi. Takip süresi ortalama 32.2 ay olup, çalışma grubumuz 16 ezotropya (%31.4), 28 ekzotropya (%54.9), dört (%7.8) multiplanar şaşılık (horizontal şaşılık ve inferior oblik hiperfonksiyonları ile birlikte "V" model olarak) ve üç vertikal şaşılık (%5.9) olgusundan oluşmaktaydı. Preoperatif olarak 28 olgumuzda (%54.9) ambliyopi ve 15 olgumuzda (%29.4) yakından Worth 4 nokta testi ile füzyon ve Titmus testi ile stereopsis (3000 ark saniye veya daha iyi) saptandı. Geri kalan 36 olguda yakından Worth 4 nokta testi ile füzyon ve Titmus testi ile stereopsis gösterilemedi. Olgularımızın hepsi genel anestezi altında opere edildi. Horizontal şaşılıklarda monolateral kısaltma-geriletme, bilateral geriletme, bilateral geriletme+faden operasyonu, "Z" miyotomi, vertikal şaşılıklarda ise alt oblik geriletmesi (tek başına veya horizontal cerrahi ile birlikte), alt oblik anteriorizasyonu veya üst rektus geriletmesi operasyonu yapıldı. BULGULAR: Postoperatif motor başarı olarak kabul edilen, primer pozisyon için horizontal planda 15 PD'den, vertikal planda 5 PD'den az deviasyona, 41 hastada (%80.3) tek seanslı cerrahi ile ulaşıldı. Preoperatif dönemde füzyon ve stereopsis sergilemeyen 36 olgunun postoperatif dönemde beşinde (%13.8) yakından Worth 4 nokta testi ile füzyon ve Titmus testi ile stereopsis (3000 ark saniye veya daha iyi) saptanması sensoriyel başarı olarak kabul edildi. Hastanın genel memnuniyeti ise (kümülatif subjektif başarı) 50 olguda (%98) temin edildi. SONUÇ: Erişkin ve çocuklarda şaşılığın karakteri ve cerrahiye cevapta farklılıklar söz konusudur. Erişkin şaşılık cerrahisi sadece kozmetik değil, fonksiyonel olumlu sonuçlar da vermektedir ve cerrahi tedavisi yüz güldürmektedir.
Abstract
PURPOSE: A retrospective analysis was conducted on adults who had had strabismus before visual maturity, and who had applied to ophthalmology clinics for treatment. We evaluated these patients operated by us, retrospectively, and discussed clinical forms, binocular visions and the results of the operations of those adults. MATERIAL AND METHODS: Fifty one adult patients with comitant strabismus [aged 16 to 47 years (mean±SD; 23±3.1)] were analysed. The average follow up was 32.2 months. Study group patients consisted of 16 cases with esotropia (31.4%), 28 cases with exotropia (54.9%), four cases (7.8%) with multiplanar strabismus (horizontal strabismus with inferior oblique overactions as "V" models) and three cases with vertical strabismus (5.9%). Preoperatively, we determined ambliyopia in 28 subjects (54.9%). Fusion by the Worth 4 dot test at near and stereopsis (3000 arc second or better) by Titmus test were recognised in 15 subjects (29.4%), remaining 36 patients had no fusion and stereopsis with the same tests preoperatively. All patients were operated on general anaesthesia. The surgical procedures performed on the patients with horizontal strabismus were unilateral resection-recession or bilateral recession or bilateral recession+faden operation or "Z" myotomy. The patients with vertical strabismus underwent either inferior oblique muscle recession with or without or together with horizontal muscle surgery or inferior oblique muscle anteriorisation or superior rectus muscle recession. RESULTS: A successful motor alignment (for the primary position, a deviation less than 15 D on the horizontal plane and a deviation of less than 5 D on the vertical plane) was achieved in 41 (80.3%) of all patients. Fusion by the Worth 4 dot test at near and stereopsis (3000 arc second or better) by Titmus test were postoperatively achieved in five (13.8 %) of 36 patients in whom binocularity had not been recorded preoperatively. Subjective success was achieved in 50 (98%) of all patients. CONCLUSION: Numerous differences exist between childhood and adult strabismus characteristics and response to surgery. It is clear that adult strabismus is more than just a cosmetic problem and treatment is worthwhile.
