Özet
AMAÇ: Cerrahi tedavi endikasyonu konulan glokomlu olgularda bir süredir uyguladığımız viskokanalostomi tekniğinin erken dönem sonuçlarını irdelemek. GEREÇ VE YÖNTEM: Glokom tanısı ile takip edilen, maksimal tedaviye rağmen göziçi basıncı 21 mmHg'nin üzerinde olan ya da topikal tedaviyi düzenli uygulamayan primer açık açılı glokom veya psödoeksfoliasyon glokomu bulunan 23 olgunun 23 gözüne viskokanalostomi uygulandı. Olgular sonuç göziçi basıncı ve kullanılan ilaçlar yönünden preoperatif dönemle karşılaştırıldı ve komplikasyonlar incelendi. BULGULAR: Yirmiüç olgunun 20'sinde postoperatif dönemde göziçi basıncı 21mmHg'nin altında idi (%86,9). Üç olguda göziçi basıncı 21mmHg'nin üzerinde tesbit edilerek beta blokör başlandı. Preoperatif ortalama göziçi basıncı 29.32 ± 8.27mmHg, postoperatif ortalama göziçi basıncı 15.72 ± 6.7mmHg idi. Böylece göziçi basıncındaki ortalama düşüş % 46.4 idi. Üç olguda Schlemm kanalı tek taraflı olarak bulunabildi, 1 olguda 10. günde kaybolan hifema, 1 olguda da blep formasyonu ile karşılaşıldı. SONUÇ: Viskokanalostominin etkili ve komplikasyonları az olan bir yöntem olması nedeniyle glokom cerrahisinde kendisine yer edinebileceği, ancak bu sonucun daha geniş hasta gruplarında uzun dönem takip sonuçları ile desteklenmesinin gerektiğini düşünmekteyiz.
Abstract
PURPOSE: To evaluate the short term results of our first viscocanalostomy operations. MATERIAL AND METHODS: We operated and followed the 23 eyes of 23 cases with primary open angle glaucoma or pseudoexfoliative glaucoma for results and complications, whose intraocular pressure values were above 21 mmHg despite maximal medical treatment or who were not cooperated to medical treatment. RESULTS: Postoperative intraocular pressure (IOP)was below 21mmHg at the 20 of the cases. Thus the success rate of this surgery was % 86.9. At the 3 of the cases IOP was above 21 mmHg and we had to begin topical beta blockers. Mean IOP were 29.32 ± 8.27mmHg and 15.72 ± 6.7mmHg pre and postoperatively. Thus mean decrease at IOP was %46.4. We could not find the one side of Schlemms channel at three of the cases. Other minor complications were; hyphema at 1 case and bleb formation at 1 case. CONCLUSION: Viscocanalostomy as an effective and safe method, may be an alternative at glaucoma surgery, but this must be supported by results of long term studies at wider patient populations.
