Laringeal maske ve trakeal entübasyon uygulamalarının göz içi basıncına etkileri

Özet
AMAÇ: Genel anestezide kullanılan laringeal maske (LM) ve trakeal entübasyon (TE) uygulamalarının göz içi basıncı (GİB), kalp atım hızı (KAH) ve sistolik kan basıncı (SKB) değerleri üzerine etkilerini karşılaştırmak. GEREÇ VE YÖNTEM: Prospektif randomize çalışmada genel anestezi altında ortopedik cerrahi girişim yapılan 20 hasta eşit olarak LM ve TE gruplarına ayrıldı. Olguların GİB, KAH ve SKB ölçümleri anestezi indüksiyonundan hemen önce, hava yolu aygıtı (HYA) yerleştirilmeden hemen önce, HYA yerleştirilmesinden 0.5 ve 5 dakika sonra, HYA'nın çıkartılmasından hemen önce ve çıkartıldıktan bir dakika sonra yapıldı. BULGULAR: İndüksiyondan hemen önceki GİB değerleri TE grubunda 16.3±3.0 mmHg, LM grubunda 17.9±2.0 mmHg idi. HYA yerleştirilmeden hemen önce GİB iki grupta da anlamlı derecede azaldı ve TE grubunda ortalama 8.4±2.8 mmHg'ya, LM grubunda ortalama 11.4±4.4 mmHg'ya düştü. GİB'ndaki bu azalma her iki grupta da indüksiyondan hemen önceki değere göre anlamlıydı (p 0.05). SONUÇ: LM'nin GİB kontrolü açısından TE'e göre daha avantajlı olduğu ve GİB artışı istenmeyen göz ameliyatlarında tercih edilebilecek bir teknik olduğu kanısına varıldı.
Abstract
PURPOSE: To compare the effects of the laryngeal mask (LM) and tracheal intubation (TI) that are used in general anaesthesia on intraocular pressure (IOP), heart rate (HR), and systolic blood pressure (SBP). MATERIAL AND METHODS: In this prospective randomized study, 20 patients who underwent orthopaedic surgery under general anaesthesia were divided into LM and TI groups equally. In all of the cases, IOP, HR and SBP were measured immediately before anaesthesia induction, before insertion of the airway device (AD), 0.5 and 5 minutes after insertion of the AD, before removal of the AD and 1 minute after removal of the AD. RESULTS: Mean IOPs were measured as 17.9±2.0 mmHg in LM and 16.3±3.0 mmHg in TI groups, immediately before anaesthesia induction. IOP significantly decreased immediately before insertion of the AD in both groups (p<0.05) and were measured as 11.4±4.4 mmHg in LM and 8.4±2.8 mmHg in TI groups. In TI group, IOP increased to 14.3±4.1 mmHg 0.5 minute after insertion of the AD and this value was significantly higher than immediately before anaesthesia induction (p<0.05), and IOP increased from 13.6±4.6 mmHg (immediately before remove of the AD) to 19.0±5.5 mmHg 1 minute after removal of the AD in TI group (p<0.05). Application and removal of LM have not caused to significantly changes on IOP (p>0.05). CONCLUSION: LM has more advantageous over TI in terms of IOP control. We suggest that it is preferable method for ocular surgery in cases whom IOP raises are unwanted.
