Multipl sklerozlu hastalarda görme sisteminin değerlendirilmesi

Özet

Amaç: Multipl Skleroz (MS) olgularında optik nevritle (ON) birlikte ya da aşikar optik nevrit görülmeksizin optik sinir lezyonu gelişebilmektedir. Bu çalışmada MS olgularında renk görme testleri, görme alanı testi (GA) ve uyarılmış görsel potansiyeli (VEP) kullanarak görme keskinliği tam olan, optik nevrit atağı öyküsü olmayan olgularda da optik sinir fonksiyonlarının bozuk olabileceğini ve ayrıca optik nevrit öyküsü olmayan ancak VEP’le optik sinirin etkilenmiş olduğu gösterilen bu olgularda renk görme testleri ve GA testlerinin etkinliğinin değerlendirmesi amaçlanmıştır. Gereç ve Yöntem: MS olgularından görsel şikayetleri olmayan ve düzeltilmiş görme keskinliği Snellen eşeline göre tam olan 17 olgunun 34 gözü çalışmaya alındı. Olgulara rutin oftalmolojik muayene, renk görme testleri (Ishihara ve F-M 100 Hue testi), VEP ve GA testi yapıldı. VEP standart kabul edilerek diğer tanı testlerinin sensivite ve spesifiteleri değerlendirildi. Bulgular: Çalışmaya alınan 17 olgunun yaş ortalaması 33.29±8.46 (22-51) yıldı. Ishihara 4 (%11) gözde, F-M 100 Hue testi 28 (%82)gözde anormal sınırlardaydı. VEP 21(%61) gözde bozuktu. F-M 100 Hue testinin sensivitesinin %100, spesifitesinin %46 olduğu; Ishihara testinin ise sensivitesinin %19, spesifitesinin %100 olduğu bulundu. Görme alanı testinin sensivitesi %86, spesifitesi %100 olarak tespit edildi. Sonuç: MS’da belirgin optik nevrit tablosu olmaksızın optik sinir tutulumu olabilmektedir. Bu çalışmada MS’lu olgularda VEP standart kabul edilerek yapılan değerlendirmede F-M 100 Hue testinin sensivitesi iyi olmasına karşın, spesifitesinin düşük olduğu; Ishihara testinin, sensivitesinin düşük, spesifitesinin yüksek olduğu; GA testinin ise hem sensivite, hem de spesifitesinin yüksek olduğu sonucuna ulaşıldı.

Abstract

Aim: Optic nerve involvement may develop in multiple sclerosis (MS) patients with or without overt optic neuritis. In this study we aimed to show that optic nerve function may be defective in MS patients with normal vision and no history of optic neuritis by performing color vision tests, visual field examinations and VEPs (visually evoked potential). The value of color vision and visual field tests for determination of optic nerve involvement in patients with MS with no history of optic neuritis whose optic nerves were shown to be effected by VEP was also evaluated. Material and Methods: Thirty four eyes of 17 patients diagnosed as MS who had no visual complaints with best corrected Snellen visual acuity of 10/10 were included in the study. Color vision (Ishihara and F-M 100 Hue test), VEP and visual field tests were done besides routine ophthalmologic examination in all patients. Sensitivity and specificity of F-M 100 Hue test, Ishihara test and visual field tests were 100% and 46 %, 19 % and 100 %, 86 % and 100 % respectively. Results: Ishihara test, F-M 100 Hue test, VEPs were abnormal in 4 (11%), 28 (82%) and 21(61%) eyes respectively. Conclusion: Optic nerve may be affected in patients with MS without overt presentation of optic neuritis. VEP regarded as the standard, F-M 100 Hue test was found to be sensitive although its specificity was low and Ishihara test was found to have low sensitivity in evaluating optic nerve function in patients with MS. Visual fields were found to be both sensitive and specific.
