Kronik blefaritte kültür ve antibiyotik duyarlılık testi sonuçları

Özet
Amaç: Kronik blefaritli olgularda konjonktiva ve kirpik dipleri mikroorganizmaları kültür ile tespit edip, bu organizmaların antibiyotik duyarlılıklarını belirlemek ve bu sonuçları kronik blefaritteki tedavi yaklaşımı yönünden değerlendirmek. Gereç ve Yöntem: Bu çalışmada kronik blefaritli 123 gözün alt forniks konjonktivası ve kirpik dibinden iki ayrı steril dakron uçlu çubukla kültür örneği alındı. Aerob ve anaerob kültürleri yapıldı, izole edilen mikroorganizmaların disk difüzyon yöntemi ile antibiyotik duyarlılıkları belirlendi. Bulgular: Kapak ve konjonktivada aynı mikroorganizma %62.6 oranında üredi. Kirpik dibinde %74.2, konjonktivada %56.9 oranında Staphylococcus aureus tecrit edildi. Konjonktivada %20.3 düzeyinde üreme olmazken, %2.4 kapak kültürü negatif sonuçlandı. Kolonizasyona konjonktiva kültürlerinde kapak kültürlerine oranla daha az rastlandı. S.aureus'un tetrasiklin direnci %47.8, tobramisin direnci %6.3, siprofloksasin direnci ise %1.3 idi. Tüm organizmaların tetrasiklin direnci %44.5, tobramisin direnci %11.0, siprofloksasin direnci ise %3.7 oranındaydı. Yüzde 100 duyarlılıkta bir ilaç tespit edilemedi. Sonuç: Çalışmamızda izole edilen mikroorganizmalar ve antibiyotik duyarlılık testi sonuçları dikkate alındığında; gerek bakterinin kendisi, gerekse ürettiği toksinlere bağlı oluşan kronik blefaritte yaygın olarak kullanılan topikal sulfonamid, aminoglikozid ve florokinolonlar şu an direnç oranları düşük gözükse de antibiyotik direnci göz önüne alınarak daha dikkatli kullanılmalı, mümkünse kültür ve antibiyotik duyarlılık çalışması ile tedavi yönlendirilmelidir.
Abstract
Purpose: To understand the etiology better and to guide the treatment of chronic blepharitis with the aid of culture and antimicrobial susceptibility test results. Materials and Methods: In this study, complete microbiological evaluation consisting of culture and antimicrobial susceptibility testing of samples obtained from the lids and conjunctiva of 123 eyes of consecutive patients with the primary diagnosis of chronic blepharitis was performed. The samples were obtained using sterile Dacron-tipped applicator and were transferred immediately to the laboratory for microbiological evaluation. Results: According to culture results growth of the same microorganisms from both the lids and conjunctiva of the same patient was obtained in 62.6% of the eyes. Staphylococcus aureus was the most frequently isolated organism (74.2% from conjunctiva and 56.9% from lid margin). There were no isolates from 2.4 % of the lid samples and 20.3% of the conjunctiva samples. The frequency of colonisation was fewer in the cultures from the conjuctiva than that of the lids. Antibiotic resistance of S.aureus was recognized in different rates for each tested drug. Tetracycline resistance was 47.8%, tobramycine resistance was 6.3% and ciprofloxasin resistance was 1.3% for S.aureus. When all the pathogens were considered, the figures were 44.5%, 11.0% and 3.7%, respectively. Conclusion: In the treatment of chronic blepharitis the antimicrobial resistance should be taken into consideration and topical sulphonamides, aminoglycosides and fluoroquinolone drops should be cautiously used. If it is possible, treatment must be prescribed after microbiological evaluation which would lead to better results
