Özet
Kapsüler blok sendromu (KBS) kapsüloreksisin nadir ve belki de bilinen tek komplikasyonudur. Bu çalışmada KBS'lu 3 olgu sunulmuştur. Olgular sırasıyla 61, 56 ve 64 yaşlarındaydı. Olgulara fako-emülsifikasyon ve katlanabilir hidrofılik göz içi lens (GİL) implantasyonu yapılmıştı. KBS, sırasıyla ameliyat sonrası 23, 17 ve 21. aylarda gözlendi. Bütün olgularda hafif ön kamara daralması, GİL'te öne doğru yer değiştirme, arka kapsülde arkaya doğru şişkinleşme, ön kapsül açıklığının lens optiği ile kapanması ve lens optiği ile arka kapsül arasına şeffaf materyal birikimi bulguları vardı. En iyi görme keskinlikleri miyopik düzeltme ile sırasıyla 0.6, 0.7 ve 0.4 idi. Olgulara neodmiyum:YAG (Nd:YAG) lazer ile arka kapsülotomi yapıldı ve kapsüler gerginlik ortadan kalktı. KBS kapsülotomi veya arka kapsülotomi yapılarak KBS başarıyla tedavi edilebilir.çoğu olguda kendiliğinden düzelmeyen bir durumdur. Nd:YAG lazer ile periferik ön kapsülotomi veya arka kapsülotomi yapılarak KBS başarıyla tedavi edilebilir.
Abstract
Capsular block syndrome (CBS) is a rare and probably the only known complication of capsulorhexis. In this study, three cases with CBS are presented here. The cases are 61, 56, and 64 years of age respectively. The patients had phacoemulsification and foldable hydrophilic intraocular lens (IOL) implantation in the capsular bag. CBS were observed at 23rd, 17th, and 21 st postoperative months respectively. MI of cases had mild shallovv anterior chamber, anteriorly displaced IOL, the posterior capsule dislended posteriorly, the anterior capsule opening was sealed to the IOL optic, and a transparent li-quefted substance accumulated betvveen the lens optic and the posterior capsule. The best corrected visual acuities were 0.6, 0.7, and 0.4 respectively with a myopic shift. The neodmiyum:YAG (Nd:YAG) laser posterior capsulotomy were performed, and the capsular distension resolved. CBS did not resolve spontaneously in most cases. The Nd.YAG laser peripheral anterior capsulotomy and posterior capsulotomy were successful in treating postoperative CBS.
