Astım Tedavisinde Kullanılan İnhalerKortikosteroidlerin ve AntihistaminiklerinOküler Yan Etkileri

Özet
Amaç: Astım tedavisinde kullanılan inhaler kortikosteroid ve oral antihistaminiklerin gözdeki yan etkilerini değerlendirmek.Gereç ve Yöntemler: Kırkbir süreğen astımlı hasta, 21 kontrol grubu, toplam 62 kişi çalışmaya dahil edildi. Hastalardan 23'ü inhaler flutikazon propionat, 15'i inhaler budezonid,3 tanesi ise her iki ilacı farklı aylarda olmak üzere beraber kullanıyorlardı. Yine hasta grubundan 19'unun astıma ek olarak rinit nedeni ile antihistaminik kullanım öyküsü mevcuttu.Tüm katılımcıların her iki gözlerinin ayrı ayrı komple oftalmolojik muayeneleri, gözyaşı kırılma zamanları (GKZ), Schirmer I testleri ve göziçi basınç ölçümleri yapıldı ve bu değerlerin sağ göz ile sol göz arasındaki ortalamaları alındı.Bulgular: Schirmer I testinde hasta ve kontrol grubu arası fark istatistiksel olarak anlamlıdeğildi (p=0,517). GKZ testinde de hasta ve kontrol grubu arası fark istatistiksel olarak anlamlı değildi (p=0,131). Göziçi basınç ölçümlerinde hasta ve kontrol grubu arası fark da istatistiksel olarak önemsizdi (p=0,242).Hasta grubundan 7'sinin sağ gözünde (%17.1), 6'sının sol gözünde (%14,6) nükleer katarakt tespit edildi (hastalardan 2'sinde bilateral katarakt mevcuttu). Kontrol grubunda isebir sağ (%4.8), bir de sol gözde (%4.8) nükleer katarakt mevcuttu.Hasta grubunda, flutikazon propiyonat kullanımı ile göziçi basınçları arasında anlamlıbir korelasyon yoktu (p=0,737). Aynı şekilde budezonid kullanımı ile göziçi basınçları arasında da anlamlı bir korelasyon görülmedi (p=0,743).Antihistaminik kullanımı ile Schirmer I testi arasında ve yine antihistaminik kullanımı ileGKZ testi arasında istatistiksel olarak anlamlı fark yoktu (sırasıyla p=0,114, p=0,075). Sonuç: Astım tedavisinde kullanılan inhaler kortikosteroidler ve oral antihistaminiklerinönerilen dozlarda kullanımının, gözdeki yan etkilerde belirgin bir artışa sebep olmadığı görülmektedir
Abstract
Aim:To evaluate ocular side effects of inhaler corticosteroids and antihistaminics usedfor the treatment of asthma.Material and Method:A total of 62 patients, 41 with persistent asthma and 21 controls,were included in the study. Twenty-three patients received fluticasone propionate, 15 patientsreceived budesonide and the remaining three patients received both agents in alternating pe-riods. In addition, 19 patients in asthmatic group received oral antihistaminic. All participi-tants underwent complete ophthalmologic examination of both eyes, tear film break up time(BUT), Schirmer I test and intraocular pressure measurement, and the average of right andleft eye values were used. Results:When Schirmer I test of patient group and control group were compared therewas no significant statistical difference (p=0.517). Differences were also not statistically sig-nificant between two groups for BUT (p=0.131) and for the intraocular pressures (p=0.242).The nuclear cataract was demonstrated in the right eyes of seven patients (17.1%) and inthe left eyes of six patients (14.6%) with asthma (Two patients had bilateral nuclear cataract)and there was cataract only in the right eyes of one patient (4.8%) and in the left eyes of one(4.8%) patient in the control group.There was not a statistically significant correlation between the intraocular pressuremeasurements in patients either using fluticasone propionate or budesonide (p=0.737 and0.743, respectively). There was not statistically significant correlation between use of antihistaminic andSchirmer I test (p=0.114). Also there was no statistically significant correlation between an-tihistaminic usage and BUT test (p=0.075). Conclusion:There is no elevation in ocular side effects due to inhaler corticosteroids andoral antihistaminics, when used for the treatment of asthma in recommended dose.
