Özet

Amaç: LASIK cerrahisi sırasında gelişen intraoperatif mikrokeratoma bağlı komplikasyonları ir­de­le­mek ve laser ablasyonun ertelendiği olgularda ikincil operasyonun güvenilirliğini ve sonuçlarını de­ğer­lendirmek. Gereç ve Yöntem: Moria M2 mikrokeratom kullanılarak LASIK cerrahisi uygulanan 338 hastanın dos­yaları retrospektif olarak incelenerek mikrokeratoma bağlı komplikasyonlar dökümante edildi. Bulgular: On hastanın 11 gözünde intraoperatif komplikasyon geliştiği görüldü (%3.25). Dört göz­de serbest flep, 2 gözde çok ince flep, 2 gözde desantralize küçük flep, 1 gözde tamamlanmamış flep ve 1 gözde düğme deliği komplikasyonu saptandı. Bir gözde yeterli vakum sağlanamadığından kesi ya­pı­lamadı. Serbest flep gelişen gözlerde operasyon aynı seansta tamamlanabilirken diğer gözlerde ope­ras­yon ileri bir tarihe ertelendi. Operasyonun ertelendiği 7 gözün 5’ine tekrar LASIK cerrahisi uy­gu­lan­dı. İkincil operasyonlarda komplikasyon görülmedi. Hiçbir gözde en iyi gözlükle düzeltilmiş görme kes­kinliğinden kayıp olmadı. Sonuç: LASIK cerrahisinde mikrokeratoma bağlı intraoperatif komplikasyonlar görülebilmekte ve bir kısmında cerrahiyi ertelemek gerekebilmektedir. Belirli bir bekleme süresi sonrasında bu gözlere y­e­ni­den LASIK cerrahisi başarıyla uygulanabilmektedir.

Abstract

Purpose: To reviewe the microkeratome related intraoperative LASIK complications and evaluate the safety and effectiveness of secondary operations in eyes in which the laser ablation was postponed. Material and Method: Charts of 338 eyes that underwent LASIK were reviewed for intra­ope­ra­ti­ve microkeratome related complications and visual outcome. Microkeratome was Moria M2 in all ca­ses. The files were examined retrospectively and microkeratome related intraoperative comp­li­ca­tions, pre­disposing factors and the results of secondary operations were documented. Results: Eleven eyes of 10 patients (3.25% ) had complications. These were free caps in 4 eyes, , de­centered small flaps in 2 eyes, extremely thin flaps in 2 eyes, incomplete flaps in 1 eye and but­ton­ho­le flap in 1 eye. In 1 eye, sufficient vacuum for flap creation cannot be reached. In eyes with free caps, la­ser operation could be done successfully. But in the rest, laser operation was postponed. Second ope­ra­tions were done in 5 of 7 eyes. Type of second operation was LASIK in all eyes. No intraoperative mi­cro­keratome related complication occured during second surgeries. None of these eyes lost any line from best correcred visual acuity. Conclusion: Intraoperative microkeratome related complications can occur during LASIK sur­gery. Some of these complications can lead to postphonement of the laser ablation. After a sufficient wai­ting period secondary LASIK surgery can be applied successfully to these eyes.
