Pop and chop nükleofraktis tekniği ile fakoemülsifikasyon cerrahisinin değerlendirilmesi

Özet

Amaç: “Pop and chop” nükleofraktis tekniği ile yapılan fakoemülsifikasyon cerrahisinin etkinlik ve güvenilirliğinin değerlendirilmesi. Gereç ve Yöntem: Fakoemülsifikasyon cerrahisinde “pop and chop” (P&C) tekniği ile ameliyat edilen 32 hastanın 40 gözü (Grup 1) ve “stop and chop” (S&C) tekniği ile ameliyat edilen 30 hastanın 40 gözü (Grup 2) karşılaştırıldı. Gruplar ameliyatta kullanılan fako zamanı (FZ), etkili fako zamanı (EFZ), toplam ameliyat zamanı (TAZ), komplikasyonlar, ameliyat sonrası EDGK, pakimetrik kornea kalınlığı açısından değerlendirildi. Bulgular: İki grup; yaş, cinsiyet, takip süresi, katarakt sertliği, ameliyat öncesi en iyi düzeltilmiş görme keskinliği (EDGK), pakimetrik kornea kalınlığı bakımından uyumlu bulundu. Her iki grup demografik özellikler ve cerrahi zorluk faktörleri açısından benzer özelliklerden oluşmakta idi ve komplikasyon oranları açısından bir farklılık yok idi. Grup 1’de FZ; 0,25±0,18 dk, EFZ 3,35±0,94 sn.,TAZ;25,12±5,12 dk idi. Grup 2’de aynı değerler sırasıyla 0,38±0,32 dk, 7,29±2,65 sn, 35,18±3,3 dk idi. Gruplar arasındaki farklılıklar istatistiksel olarak anlamlı idi (p 0,05). Ameliyat sonrası EDGK sırasıyla 0,76±0,9 ve 0,79±0,11 idi (p>0,05). Sonuç: P&C tekniği ile fakoemülsifikasyon cerrahisi S&C tekniği ile güvenirlik yönünden benzemekte olup, kullanılan fako enerjileri ve zaman açısından daha üstündür.

Abstract

Aim: To evaluate the safety and effiacy of &#8220;pop and chop&#8221; phacoemulsification technique. Material and Method: 40 eyes of 32 patients with &#8220;pop and chop&#8221; technique (Group 1). were compared with 40 eyes of 30 patients on in which the &#8220;endocapsular stop -chop &#8220; tecnique was used (Group 2). The groups were compared with regard to the phaco time used during surgery, efective phaco time, total operation time, complications, postoperative BCVA and pachymetric differences. Results: The two groups were found to be statistically compatible in terms of age, gender, follow up period, cataract hardness, preoperative best corrected visual acuity (BCVA), pachymetric corneal thickness. The two groups were similar in demographic features and surgical diffucilty factors. There were no difference in the complications rate. The phaco time (mean: Group 1, 0,25±0,18 minutes (SD), Group 2 0.38±0.32 minutes), effective phaco time (3,35± 0,94 seconds and 7,29± 2,65 seconds), and total operation time (25.12±5.12 minutes and 35,18±3,3 minutes) were significiantly lower in Group 1 than Group 2 (p<0.05). In 4 cases (%10) of in Group 1 and in 6 cases (%15) of in Group 2 intraoperative complications were encountered (p>0.05). Postoperative BCVA were 0,76±0.9 and 0,79±0,11 respectively (p>0.05). Conclusions:&#8221; Pop and chop&#8221; nucleofaractis tecnique was found to be as safe as the standart endocapsüler phaco tecnique but superior for in terms of the phaco time and energy used.
