Kornea yüzeyi onarımında amniyon zarı transplantasyonunun klinik sonuçları

Özet

Amaç: Kornea yüzeyi onarımında amniyon zarı transplantasyonunun (AZT) etkinliğini araştırmak. Gereç ve Yöntem: Ocak 2005-Şubat 2010 tarihleri arasında Kocaeli Üniversitesi Tıp Fa- kültesi Göz Hastalıkları Polikliniği’ne başvuran ve farklı kornea yüzey hastalıkları nedeni ile taze AZT uygulanan 43 hastanın 43 gözü çalışmaya alındı. Bu retrospektif çalışmada hasta- ların tanılarına, preoperatif ve postoperatif düzeltilmiş en iyi görme keskinliklerine (DEİGK), amniyon zarı atılma sürelerine ve kornea epitelizasyonu tamamlanma sürelerine; korneal ül- ser olgularında kültür örneklerine ve sitoloji sonuçlarına bakıldı. Bulgular: Olguların 16’sı (%37.2) kadın, 27’si (%62.8) erkek, ortalama yaşı 54.1±17.2 idi. Desmatosel, korneal erime ve ülserli iyileşmeyen epitel defekti olguları Grup 1’de (n=27); nö- rotrofik keratit ve ülsersiz iyileşmeyen epitel defekti olguları Grup 2’de (n=16) toplandı. Grup 1’deki gözlere doldurma, greft ve örtü tekniği ile Grup 2’deki gözlere çok katlı örtü tekniği ile AZT yapıldı. Hastaların postoperatif DEİGK preoperatif DEİGK’den yüksekti (p=0,01). Am- niyon zarının atılma süresi ortalama 2.7±0.8 hafta, kornea epitelizasyonun tamamlanma süre- si ortalama 7.7±5.8 haftaydı. Sonuç: Kornea yüzeyi onarımında AZT etkili, hazırlanması kolay, maliyeti düşük bir teda- vi yöntemidir.

Abstract

Aim: To evaluate the effectiveness of amniotic membrane transplantation (AMT) for re- construction of corneal surface. Material and Method: Fourty tree eyes of 43 patients who were applied AMT due to the different corneal surface diseases were included in the study. In this retrospective study, the di- agnoses, preoperative and postoperative best corrected visual acuities (BCVA), durations of amniotic membrane disruption, corneal epithelialization times, the results of corneal ulcer cul- ture and cytology were examined. Results: Sixteen patients (37.2 %) were female and 27 (62.8 %) were female. The mean age was 54.1±17.2. Eyes with descemetocele, corneal melting and persistent epithelial defect with ulceration were classified as Group 1 (n=27); eyes with neurotrophic keratopathy and per- sistent epithelial defect without ulceration were classified as Group 2 (n=16). AMT was ap- plied to the Group 1 with fill in, graft and patch technique together. In Group 2, AMT was ap- plied with multilayer patch technique. Postoperative BCVA of patients are higher than preop- erative BCVA (p=0.01). The mean duration of amniotic membrane disruption was 2.7±0.8 weeks, the mean of corneal epithelialization time was 7.7±5.8 weeks. Conclusion: AMT is effective, easy to prepare, low-cost method for the treatment of corneal surface reconstruction.
