Üçüncü sinir felcinde etyolojik klinik değerlendirme ve tedavi yaklaşımı

Etiology clinical features and strategies of treatment in third nevre palsy

Özet

Amaç: Üçüncü sinir felci nedeniyle takip edilen olgularda etyolojik nedenleri, klinik özel- likleri incelemek ve uygulanan tedavi yaklaşımlarını değerlendirmek. Gereç ve Yöntem: Ocak 1995-Aralık 2010 tarihleri arasında Sağlık Bakanlığı Ankara Eğitim ve Araştırma Hastanesi şaşılık bölümünde üçüncü sinir felci nedeniyle takip edilen 15 olgunun dosyaları geriye dönük olarak incelendi. Olgular etyolojiye yönelik olarak ayrıntılı anamnez, laboratuvar ve görüntüleme tetkikleri ile değerlendirildi. Olgular klinik olarak ver- tikal ve horizontal kayma miktarları, oblik ve horizontal kas fonksiyonları, baş pozisyonu ve diplopi varlığı, binoküler görme ve stereopsis düzeyleri, uygulanan tedavi yaklaşımları açı- sından değerlendirildi. Cerrahi geçiren olgularda cerrahi başarı, 10 prizma diyoptri (PD) ve altı horizontal kayma olarak tanımlandı. Bulgular: Üçüncü sinir felci nedeniyle takip edilen 15 olgunun yaş ortalaması 23,8±20,2 idi. Etyolojik olarak 6 olguda (%40) konjenital, 4 olguda (%26,7) idyopatik, 2 olguda (%13,3) vasküler, 2 olguda (%13,3) travmatik, 1 olguda (%6,7) neoplastik nedenler saptandı. Beş has- tada (%33,3) pupil tutulumu mevcuttu. Olguların 6’sına (%40) konservatif tedavi, 9’una (%60) cerrahi tedavi uygulandı. Cerrahi sonrası ekzotropyadaki değişim ile iç ve üst rektus kas fonk- siyonlarındaki değişim anlamlı iken (p<0,0001, Wilcoxon testi) hipotropya miktarındaki deği- şim anlamlı değildi (p=0,068, Wilcoxon testi). Cerrahi uygulanan 9 olgunun 6’sında cerrahi sonrası ekzotropya 10 PD ve altında olup cerrahi başarı oranı %66,6 idi. Sonuç: Bir ya da daha fazla kası etkileyecek tarzda kısmi veya tam felç şeklinde görülebi- len 3. sinir felci pek çok sistemik veya nörolojik hastalığın ilk ortaya çıkış bulgusu olabileceği için ayrıntılı anamnez ve muayene ile etyolojiyi belirlemek önemlidir. Uygun medikal, cerrahi tedavi ve yakın takip ile başarılı sonuçlar elde edilebilmektedir.

Abstract

Aim: In this study we aimed to investigate the etiologic factors related to third nerve palsy and clinical features of these patients and also to assess treatment approaches to the disorder. Material and Method: The medical records of 15 patients, who were referred to Ophthalmology Department of Ministry of Health Ankara Training and Research Hospital be- tween January 1995- December 2010 with third nerve palsy were investigated retrospectively. The etiological assessments were performed by detailed medical history, laboratory measure- ments and imaging techniques. The patients were evaluated with respect to their vertical and horizontal deviations, oblique and horizontal muscle functions, head position and presence of diplopia, binoculer vision and stereopsis levels and applied treatment approaches. Success of surgery was defined as 10 PD or less horizontal deviations for the patients who underwent sur- gery. Results: The average age of 15 patients with third nerve palsy was 23.8±20.2 years. Etiologic factors were congenital for 6 patients (40%), idiopathic for 4 patients (26.7%), vas- cular for 2 patients (13.3%), traumatic for 2 patients (13.3%) and neoplastic for 1 patient (6.7%). Five patients (33.3%) had pupil involvement. Treatment approaches were conservative for 6 patients (40%) and surgical for 9 patients (60%). Change of exotropia and functions of medial and superior rectus muscle after surgery were found to be significant (p<0.0001, Wilcoxon test), where change in hypotropia was not (p=0.068, Wilcoxon test). In six of nine patients who underwent surgery, exotropia was found to be 10 PD and less after surgery and success rate of surgery was 66.6%. Conclusions: As partial or total third nerve palsy that affects one or more muscles could be the first sign of many systematic or neurologic diseases, etiology must be defined with de- tailed medical history and examination. Successful results could be achieved by appropriate medical treatment and surgery.
