Prophylactic Laser Photocoagulation Following Scleral Fixated Intraocular Lens Implantation

Özet

Amaç: Skleral fiksasyonlu göziçi lensi (SFGİL) implantasyonu sonrası yapılan koruyucu Argon lazer fotokoagülasyon (LFK)'un, retina dekolmanı (RD)'nı önlemedeki etkisinin araştırılması.Gereç ve Yöntem: SFGİL implantasyonu yapılan 82 hastanın 82 gözü çalışmaya dahil edildi. Ameliyat öncesi tanılar; afaki 15 (%18,29)hasta, komplike katarakt 28 (%34,15) hasta,"drop nükleus" veya travmatik lens luksasyonu 30 (%36,58) hasta, arka kamara lensi sublüksasyonu 9 (%10,98) hasta olarak saptandı. Tüm hastalara; SFGİL implantasyonundan 3 hafta sonra, 360oüç sıra koruyucu LFK yapıldı. Bulgular: Ortalama takip süresi 7,01 aydı. Ameliyat sonrası 6. ayda, en iyi düzeltilmiş görme keskinliği (EDGK); hastaların 5 (%6,1)'inde artmazken, 77 (%93,9) hastada arttı. SFGİL implantasyonunda, intraoperatif komplikasyon olmadı. SFGİL implantasyonu sonrası;vitreus hemorojisi [2 (%2,44) hasta], hifema [3 (%3,66) hasta], GİL desantralizasyonu [2 (%2,44) hasta], GİL tilti [1 (%1,22) hasta], geçicikornea ödemi [5 (%6,1) hasta], geçici göziçi basıncı yükselmesi [4 (%4,88) hasta], sütür iritasyonu [3 (%3,66) hasta] oldu. Hastaların hiçbirinde RD gelişmedi. Sonuç: SFGİL implantasyonu sonrası yapılan koruyucu Argon LFK, RD gelişimini önlemede etkin olabilir

Abstract

Objective: To investigate the effectiveness of prophylactic Argon laser photocoagulation (LP) in prevention of retinal detachment (RD) following scleral fixated intraocular lens implantation (SFIOL). Material and Method: This retrospective study included 82 eyes of 82 patients. Preoperative diagnosis were as follow: aphakia 15 (18.29 %) patients, complicated cataract 28 (34.15%) patients, dropped nucleus or traumatic lens luxation 30 (36.58%) patients, subluxated IOL 9 (10.98%) patients. Three weeks after SFIOL implantation, 3 rows of prophylactic Argon LP was applied to 360o peripheral retina, in all patients. Results: The mean follow up was 7,01 (6-30) months. At post-operative 6th month, the best corrected visual acuity (BCVA); did not changed in 5 (6.1%) patients, increased in 77 (93.9%) patients. Following the SFIOL implantation; vitreous haemorrhage [2 (2.44%) patients], hyphema [3 (3.66%) patients], IOL decentralisation [2 (2.44%) patients], IOL tilt [1 (1.22%) patients], transient corneal oedema [5 (6.1%) patients], transient increased intraocular pressure [4 (4.88%) patients], suture irritation [3 (3.66%) patients] were observed. RD was not observed in any patients. Conclusion: Prophylactic Argon LP following SFIOL implantation may be effective in prevention of RD.
