Fakoemülsifikasyonun diyabetik retinopati progresyonu üzerine etkisi

Özet
Amaç: Fakoemülsifikasyonun diyabetik retinopati (DR) seyrine etkisini belirlemek amaç- landı. Gereç ve Yöntem: Ankara Atatürk Eğitim ve Araştırma Hastanesi Göz kliniği retina bölü- münde DR tanısıyla takip edilen ve Ocak 2005 - Aralık 2009 yılları arasında tek gözlerine fa- koemülsifikasyon uygulanan 53 hastanın 106 gözü çalışma kapsamına alındı. Hastaların ope- re edilmeyen gözleri kontrol grubunu oluşturarak cerrahinin DR prognozuna etkileri incelen- di. Hastaların postoperatif 1. günde, 1. haftada, 1- 6 ve 12. aylarda kontrol muayeneleri ya- pıldı. Cerrahiden 1 ay önce ve 1 ay sonrası dönemde fotokoagülasyon uygulananlar, katarakt ameliyatı dışında oküler cerrahi öyküsü olanlar ve katarakt cerrahisinde arka kapsül yırtıl- ması gelişen olgular çalışmaya dahil edilmedi. Başarılı bir cerrahi operasyon geçirmiş olan hastalar çalışmaya alındı. Bulgular: Fakoemülsifikasyon cerrahisini takiben opere olan 23 (%43,3) gözde DR prog- resyonu izlenirken, opere olmayan gözlerin 18 (%33,9)inde DR progresyonu görüldü (p<0.05). Hastaların cinsiyet, yaş ve HbA1c özellikleri açısından DR progresyonunda fark bu- lunmadı. Diabetes Mellitus (DM) süresi DR progresyonu olanlarda ortalama 11.34 yıl, prog- resyon olmayanlarda 7,71 yıl idi. Erkek ve kadınlar arasında DR progresyonu açısından fark bulunmadı. Sonuç: Diyabetik retinopatili hastalarda fakoemülsifikasyon cerrahisinin DR progresyo- nunda etkili olduğu sonucuna varılmıştır.
Abstract
Aim: To determine the effect of phacoemulsification on progression of diabetic retinopathy. Material and Method: 106 eyes of 53 consecutive patients having DR and undergone pha- coemulsification in one eye between January 2005 and December 2009 at retina department of Ataturk Training and Research Hospital were included in this study. The fellow eyes of the patients composed the control group so the effect of cataract surgery was investigated on prog- nosis of diabetic retinopathy . Patients were seen on the first day, end of the first week, first, sixth and twelfth months post- operatively. Patiens having laser photocoagulation one month before or after the surgery, pa- tients having ocular surgeries other than phacoemulsification and patients whom their poste- rior capsule was opened during phacoemulsification were excluded from the study. Patients with successful phacoemulsification were included in the study. Results: Diabetic retinopathy progression was seen in 23 (%43.3) eyes with phacoemulsi- fication and in 18 (%33.9) eyes without phacoemulsification (p<0.05). There was no difference among sex, age and HbA1C regarding progression of diabetic retinopathy. Mean time of dia- betes in patients with progression of diabetic retinopathy was 11.34 years, whereas in patients whithout progression it was 7.71 years. There was no difference regarding diabetic progres- sion among males and females. Conclusion: In conclusion, phacoemulsification affected diabetic retinopathy progression in patients with diabetic retinopathy.
