Kuru Göz Hastalarında Nötrofil Lenfosit Oranının Oküler Yüzey Bulguları ile İlişkisi

Özet
Amaç: Primer ve sekonder Sjögren Sendromu'na bağlı kuru göz sendromu olan olgularda, sistemik enflamasyon belirteci olarak nötrofillenfosit oranını değerlendirmek ve nötrofil-lenfosit oranının oküler yüzey boyanma bulguları ile korelasyonunu incelenmek.Gereç ve Yöntem: Primer Sjögren Sendromu tanısı olan 24 olguda, romatoid artrite bağlı sekonder Sjögren Sendromu tanısı olan 24 olguda ve 20 sağlıklı olguda serum nötrofil-lenfosit oranı ve C-reaktif protein seviyeleri değerlendirildi. Tüm hastalarda Schirmer I testi, gözyaşı kırılma zamanı, oküler yüzey boyanma dereceleri değerlendirildi.Bulgular: Her üç gruptaki demografik özellikler benzerdi. Ortalama nötrofil-lenfosit oranını değeri, primer Sjögren Sendromu grubunda2,7±0,6 (0,95-3,8); sekonder Sjögren Sendromu grubunda 2,6±0,7 (0,8-3,4); kontrol grubunda 1,7±0,8 (0,89-3,9) idi (p=0,02). Ortalama C-reaktif protein düzeyleri, primer ve sekonder Sjögren Sendromu gruplarında, kontrol grubuna göre daha yüksek tespit edildi (p=0,03). Primer vesekonder Sjögren Sendromu gruplarında, nötrofil-lenfosit oranının değerleri ile oküler yüzey boyanma dereceleri arasında pozitif korelasyonsaptandı (Primer Sjögren Sendromu: r=0,20, p=0,04; sekonder Sjögren Sendromu: r=0,34, p=0,01).Sonuç: Nötrofil-lenfosit oranını, primer ve sekonder Sjögren Sendromu'nda kontrol grubuna göre yüksek idi. Nötrofil-lenfosit oranı düzeyleri ile oküler yüzey boyanma dereceleri arasındaki pozitif korelasyon, sistemik enflamasyonla birliktelik gösteren bu otoimmün hastalıklarda, nötrofil-lenfosit oranının oküler tutulum ile ilişkili olabileceğini düşündürmektedir
Abstract
Objective: To evaluate the neutrophil-to-lymphocyte ratio as a systemic inflammation marker and the relationship between neutrophil-tolymphocyte ratio and ocular surface staining scores in patients with primary and secondary Sjögren syndrome. Material and Method: Serum neutrophil-to-lymphocyte ratio and C-reactive protein levels were evaluated in 24 patients with primary Sjögren Syndrome, 24 patients with secondary Sjögren Syndrome associated with rheumatoid arthritis and 20 healthy controls. Schirmer I test, tear film break-up time and ocular surface staining scores were obtained for all patients. Results: Demographic properties were similar in 3 groups. The mean neutrophil-to-lymphocyte ratio value was 2.7±0.6 (0.95-3.8) in primary Sjögren Syndrome group, 2.6±0.7 (0.8-3.4) in secondary Sjögren Syndrome group and 1.7±0.8 (0.89-3.9) in the control group (p=0.02). The mean C-reactive protein levels were statistically higher in primary and secondary Sjögren Syndrome groups when compared to controls (p=0.03). In primary and secondary Sjögren Syndrome grups, there were positive correlation between neutrophil-to-lymphocyte ratio values and ocular surface staining scores (Primary Sjögren Syndrome: r=0.20, p=0.04; secondary Sjögren Syndrome: r=0.34, p=0.01). Conclusions: Neutrophil-to-lymphocyte ratio was higher in primary and secondary Sjögren Syndrome groups when compared to controls. The positive correlation between neutrophil-to-lymphocyte ratio and ocular surface staining scores suggests that neutrophil-to-lymphocyte ratio may be related with ocular involvement in these autoimmune diseases which are associated with systemic inflammation.
